If i have a lung nodule 4 mm non calcified that remain stable onfollowup ct scan could that cancer?
Could that metasisise to tibia?

If the nodule is stable, just 4 mm, not progressed on CT scan, the possibility of cancer is least.
Your another query says: could that metastatize to tibia:
If all the nodule is due to cancer, it can spread to any part of the body. The lung cancer or for that matter any organ with high blood supply can spread via blood stream.
CT scan is usually indicative of what the disease may be and it is always the histo-pathological diagnosis then only we can say it is cancer or not.
If you have any symptoms related to tibia, you need confirmatory CT/MRI scans and FNAC or biopsy to say whether this is secondary or not.

Im confused if a 4 mm nodule remained stable, after 10 nonths time, there is still a chance it is a cancerous nodule, yes?

If thiis is a possibility, and i am asked to go back for another ct scan in 18 months, what is likelihood it will spread while waiting and watching?

Least possible.

10 months is a long time for any nodule to remain stationary/same - hence most probably a benign problem like a granuloma and most probably not a cancerous growth.
Scan after 18 months is done to see the progress.
In the meantime other symptoms to be watched:
Loss of appetite, weight, coughing, blood in coughing and any other changes that one notices.
