Brief Answer:
Probably inner ear dysfunction. Read below.

Detailed Answer:
Hello and welcome to "Ask a Doctor " service.

I have read your query and the MRI report that you uploaded.

Among the findings of the MRI that you uploaded, pansinusitis may result in vertigo by affection of the middle and inner ear.

The report states that there is ectopia of cerebellar tonsils, I think that is an incidental finding since there are no Chiari malformation symptoms and the ectopia is smaller than 5 mm.

Periodic follow up with MRI (once a year) or in case of new neurological symptoms, is sufficient.

Hyperintensities in the MRI seems to be linked with the unstable blood pressure that has damaged small brain vessels, and correct control of blood pressure is necessary to prevent worsening of this condition.

In conclusion, I think that your symptoms are linked with sinusitis or other conditions such as benign paroxysmal positional vertigo,?etc.

Consider consulting with an ENT Doctor for your issues.

Hope you found the answer helpful.

Regards!
