Brief Answer:
heart ultrasound required

Detailed Answer:
Hi and welcome.
These symptoms are not very specific and considering your age, I dont think that some ischemic heart issues can be present, but some types of arrithmias may anifest on this way so ultrasound of heart would be recommendable.
However, in young people it is often related to anxiety or there may be some non-thoracic issues such as hiatal hernia, cervical spine disorders or circulation disorders in affected arm. If heart results are fine, you should focus on treating anxiety, have healthy diet, and reduce weight if overweight. Wish you good health. REgards
