Hi, good ultrasound machines can detect the heart pulsation at 6 weeks of pregnancy, sometimes detected by 8 weeks. Get one more ultrasound done to confirm absence of cardiac activity and then your doctor may decide to terminate the pregnancy keeping your wife health as priority. Spotting may occur in pregnancy due to various reasons. Your doctor may suggest some tests to find the reason for this.

Consult an Infertility specialist. Get partner's semen analysis. ALso, follicular monitoring to check for ovulation and hormonal tests. Get laparoscopy done to heck for patency of fallopian tubes.
 
Hello Doctor, I received a report of NT scan & it is as follows. Fetal movements& cardiac activity normal. Placenta is anterior & grade 1 in maturity. No evidence of placenta prevail or retro placental haemorrhage. Heart rate: 150 beats/ min NT measures: 1.1 mm Nasal bone noted Crown rump length measures: 6.33 cm, 13 weeks 0 days I am concerned about the grade 1 placenta maturity..
