What Causes Fatigue And Tiredness?

I have gone to my complaining of always being fatigued and tired no matter how much I sleep and found that my testosterone level way extremely low so he put me on steroid injections which have helped very little and the more I look into my symptoms I believe I have ES with narcolepsy. I went back to Dr. and he for some reason thinks I have insomnia which is completely opposite according to the symptoms and he refused to give me adderall which I have tried once and it was the first time I have felt normal in years. How should I approach him with this to make him understand?
