What Causes Abdominal Discomfort With Belching And Bloating?

Hi First i want to thank you about the time you give to people for the problems they have. I am a 22 years old boy and for 4 years have had a pain which started as an abdominal pain and than it grew worse and the pain was located to my back. That made me go to a neurologist who made me some tests and told that it was propably due to training in the gym.He told me that perhaps it was radiculitis and gave me some anti inflamatory drugs but pain didn t get better at all,but contrary to that it grew worse and the culmination point was at the the middle of the night.Than i went to a gastroenterologist .He made me a fibrogastroduodenoscopy and said that i suffer from an old ulcer in the bulb of the duodenum. The diagnosis written in the letter was as below: *PASSAGE TILL THE BULB OF THE DUODENUM.CAN NOT PAS BEYOND BECAUSE OF A DEEP ANTERIOR ULCER(about 1.5 cm) COVERED WITH FIBRINE,WICH CAUSES A PARTIAL STENOSIS. *THE STOMACH HAS NO LESIONS. *HIPEREMIC EROSIVE MUCOSA IN THE 1/3 OF THE INFERIOR ESOPHAGUS CONLUSION:-CHRONIC ULCUS BULBI DUODENI IN ACTIVITY WITH PARTIAL STENOSIS -EROSIVE ESOPHAGITIS TREATMENT:ESOMEPRAZOL(20 mg) 2 TIMES A DAY 30MIN BEFORE MEALTIME(1 tablet in the morning and 1 tablet at night)FOR 42 DAYS. TOTAL NUMBER OF TABLETS=84(3 PACKS) RICONTROL AFTER 1 MONTH I took the drug as described and the symptoms evanished immediately.Than after 1 month went again for a recontrol.I did again the fibrogastroduodenoscopy and the result was as below: *PASSAGE TILL THE BULB OF THE DUODENUM. *THE BULB OF THE DUODENUM IS DEFORMATED BECAUSE OF AN ANTERIOR ULCER WHICH HAS CAUSED A STENOSIS IN THIS LEVEL. *ANTRUM,CORPUS,FUNDUS,CARDIA WITH NORMAL MUCOSA AND NO LESIONS. *HIPEREMIC MUCOSA IN THE 1/3 OF THE INFERIOR ESOPHAGUS CONCLUSION:-ULCUS BULBI DUODENI WITH A PARTIALLY CICATRIX WITH PARTIAL STENOSIS -PEPTIC ESOPHAGITIS TREATMENT:ESOMEPRAZOL(20 mg) 2 TIMES A DAY 30MIN BEFORE MEALTIME(1 tablet in the morning and 1 tablet at night)FOR 84 DAYS. TOTAL NUMBER OF TABLETS=168(6 PACKS) *ENDOSCOPIC RECONTROL AFTER 3 MONTHS I continued the cure as the doctor described me but i always was wondering to my self why he did nt give me any antibiotics.Anyway i continued taking the drugs with regularity.I was ok and had no symptoms. After 84 days went to my docor for the recontrol.I did again fibro gastro duodeno scopy and the result was as below: *PASSAGE TILL THE SECOND SEGMENT OF THE DUODENUM WICH IS NORMAL. *THE BULB OF THE DUODENUM IS DEFORMATED BECAUSE OF AN ULCER PARTIALLY EPITHELIZED IN THE ANTERIOR FACE WICH HAS CAUSED A STENOSIS IN THIS LEVEL. *ANTRUM,CORPUS,FUNDUS,CARDIA WITH NORMAL MUCOSA AND NO LESIONS. *ESOPHAGUS WITH NORMALL MUCOSA AND NO LESIONS. CONCLUSION:ULCUS BULBI DUODENI PARTIALLY EPITHELIZED WITH A PARTIAL STENOSIS. TREATMENT:ESOMEPRAZOL(20mg)ONCE A DAY IN THE MORNING FOR 56 DAYS.TOTAL NUMBER OF TABLETS=56(2 PACKS) CLINICAL RECONTROL AFTER 56 DAYS. Know,i am concerned about the fact that he didn t gave me any antibiotics. When i asked him ,he didn t reply to me but told me to take what he said.i am still using the drugs and there are 28 days left before i go see again my doctor for a clinical control, but it has been a couple of days that i see that i am not feeling well.I don t have pain but some gut disorders and defecationI have bloating ,gas and belching very much.today, when i saw my defecation,it seemed vey black and i also have had an abdominal discomfort.I am being concerned about the fact that my ulcer might have recurrent.Should i start any antibiotics? Please anybody give me a good answer. Thank you very much for your disponbillity. Studenti

General Surgeon 's  Response
Hello

* As per my surgical experience, the chronic discomfort is likely due to narrowing of the first part of the duodenum which co-relate with the child age.

* As the repeated endoscopies are consistent with the same finding of narrowing and deformity, an MRI of the abdomen would serve best to look further to figure out the final course of cure.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr Bhagyesh V. Patel, General Surgeon
