Hi,

While taking hormonal contraceptive pills, inter-menstrual spotting can be seen as brown discharge sometimes, due to fluctuations in the hormone levels. Accutane contains isotretinoin, and it is known to affect the function of the birth control pills. And this could be one reason of the withdrawal effect on the endometrium and spotting. You have not mentioned whether you have started the second birth control or not. Because that may also affect the function of these birth control pills.

Usually minimal brown discharge can subside soon without need of treatment. If persists, better to consult your gynecologist once to find out the possible cause and treatment can be planned accordingly. Usually antifibrinolytic drugs and increasing the dose of hormonal contraceptives can help in this type of situation.

Hope I have answered your question. Let me know if I can assist you further.

Regards,
