Diabetologist 's  Response
Dear sir I understand your concern.I am sorry to hear you have been having difficulties getting your blood sugar in the normal range now. I am very happy you are this conscientious because having a well controlled blood sugar is very important in preventing the onset of diabetic complications.
After reading your entire history i understand that, there are multiple problems need to be addressed
1.kindly check for cardiac condition with ECG and ECHO,To make sure that there is no problem in heart
2.medication what you are on is fine but you may need to change your mixtard to novorapid and lantus for better control and to avoid low sugars.
3.daily go for mild exercise like walking which will give you good diabetes control and strengthens your bones also.
4.follow good diet pattern consult good diet expert for diet plan.
I suppose I have given the desired response if you have any other query please write back.
Otherwise please accept & rate my answer. I will appreciate your review about my answer.
