OBGYN, Maternal and Fetal Medicine 's  Response
Hi, I have gone through your question and understand your concerns. Herniation of sac with delivery at 20 weeks of pregnancy is suggestive of cervical incompetence in cervix dilates very early in pregnancy leading to delivery in second trimester.
Placental hematoma can be an associated finding, it generally do not lead to early dilatation of cervix and delivery.
I would suggest you to consult an OBGYN specialist to get properly examined and investigated to confirm cervical incompetence. If it is present, then you need to get a cervical encerclage stitch at 14-16 weeks in next pregnancy to prevent premature delivery.
Hope you found the answer helpful.
Wishing you good health.

Dr Deepti verma
