Head Injury Due To Fall. Headache, Nausea, Sore Scalp, Tiredness. Concussion?

Four days ago, I turned clumsily while rising from the floor (which I was cleaning), and accidentally whacked the right temporal area of my head on a flat towel rack that was in the wall. For a short time (1 minute or so), I felt like my "bell had been rung," but didn't lose consciousness, and there was no laceration. About 4 hours later, I developed a headache on right side of head, and felt nauseous, and a little discombobulated for an hour or so. It was late at night and no doc was available, so I stayed up all night (out of concern that I might have a epidural or subdural bleed). Next day, scalp was very sore, but just minor bumps were palpable. I iced the area off and on for the first 24 hours after injury. Since then, scalp very sore, I have been very tired, with intermittent headache and occasionally, a little mental confusion which is not typical of me. Now the area over my right cheekbone also hurts, along with temporal pain. No other symptoms of TBI in the last 4 days. I feel anxious about the possibility of brain injury, but currently have no insurance and am unemployed, so haven't wanted to go to hospital and incur medical bills if unnecessary. Does this sound like a mild concussion, or do you think a CT scan is in order? Thanks very much for your help!

Hi ! good evening. I am Dr Shareef answering your query.

It could be just the effects of a concussion following the injury. The local pain in the scalp along with the feeling of bump could be due to a small hematoma formation with inflammation. Once the initial 24 hours are over, the chances of any major intracerebral pathology becomes less, and a wait and watch policy is acceptable. One has to of course be careful enough to watch any change in the level of consciousness in you, and if this happens,you have to consider going for a CT scan to rule out any intracranial hematoma formation.
Till then, you can go for some kind of anti inflammatory drug along with a proton pump inhibitor to reduce the bumps and pain in your scalp.
