Could Ongoing History Of Constipation Be A Sign Of Anal Fissure And Faecal Impaction?

female, 33, 11stone, ongoing history of constipation since aged 8, only ever have a bowel movement once / twice a week for 25 years, had large bowel movement a few days ago and now have razor blade like pains during bowel moment, some fresh blood, had this before and it went away, but also now experincing pressure like sensation around anus and perenium, as if something is pushing downwards and outwards. Is this an anal fissure or faceal impaction?

As per your complain you have the symptoms of chronic constipation..

Bleeding during bowel evacuation and razor blade like pains can be due to tear in the anal mucosa known as Anoderm leading to Anal Fissure and occurs due to injury to the mucosa caused by very hard stools that are common occurrence in constipation..

Feeling of something pushing down along with Faecal Impaction can be most probably due to faecal impaction and intestinal gas formation due to constipation..
Pain and pressure can also be felt due to fissure formation..

Constipation can be due to causes like Irritable bowel syndrome, excess intake of dairy,Eating disorders, Lack of appropriate physical activity, side effects of medicines, Hypothyroidism, Lazy bowel syndrome, damaged nerves of digestive tract etc..

I would suggest you to consult a Gastroenterologist and get evaluated and he can advise you get investigations done like Colonoscopy, Barium swallow, hormonal assy etc to confirm the cause..
Treatment of the cause can relieve the symptoms..
You can also be advised to take laxativs like Polyethylene glycol [Miralax], castor oil also acts as a lubricant and laxative..
You can also be advised to take Psyllium husk or Lactulose..
Take more fibres in diet, plenty of water and do moderate workout..
You can also be advised medications like Lubiprostone or Linaclotide..

For anal fissures you can be advised to apply topical anaesthetics like Lignocaine gel..
Calcium channel blocker containing ointments like Diltizem can be advised as it will dilate the blood vessels in the area and improve blood flow that will initiate healing..
