What Does This Blood Report Indicate?

Question: I have just had a blood test after feeling quite lightheaded and tired, with a very mild stomach ache, for over ten days. The results were all in the normal range, apart from the oesinopil count, which was 11%. Is this a worry? The symptoms are still there, although they do seem to fluctuate.

Hi I did review your concern.

High eosinophil count in this range can be due to allergies or recent parasitic infections in bowel or lungs or anywhere else to rarely some blood events that we need to rule out.

1)Have you noticed any fever in past few days?
2)Have you noticed any changes in your bowel habits in past few days?
3)Have noticed any other problems apart from stomach ache and lightheadedness in past few days like coughing, chest pain, shortness of breath, weight loss, low appetite?
4)Have you been to anywhere else apart from your home and work in past few days? And where do you live?

-Can you send me the complete blood report just to see the trends of other counts even though they are normal?

My next advice for investigations would be stool for ova and cysts since you have had a stomach ache and also repeat the blood test in 2 weeks time to see how the eosinophil count goes.

In regard to your questions:
1/No, I haven't had any fever. My temperature is normal (36-5).
2/Not really, although I do seem to be passing stool more regularly than usual.
3/ None of the mentioned symptoms, but i do have a light headache. It's a feeling of being very 'foggy', the type of feeling you might expect with a mild flu, without the other usual flu symptoms cough etc.
4/I am currently living in Lebanon, since last summer. We live in London and i do travel back and forward between the two places. I have recently returned from a trip to XXXXXXX The symptoms described only appeared after I returned to Beirut.

I did review your reply and thank you for sending me the report.

Can you please tell me what are the units mentioned for bilirubin report and indirect bilirubin report?
Your blood counts seem to be fine.
But since you are having some discomfort and flu like symptoms I would recommend a blood report with a peripheral smear view by a hematopathologist just to be sure about the morphology and cell count.

My other recommendation would be stool test for ova and parasites so that if thats positive you can be treated with some antihelminthics and your eosinophil count will come down. Since you live in lebanon which has a mild to moderate parasite prevalence rate it could be a mild bowel infection with any of the helminithic agents and nothing much to worry about.

SO all in all
1) Stool test for ova and parasites.
2) Repeat blood count in 1-2 weeks with a peripheral smear to be examined by a hematopathologist.
3) Also please let me know what is the unit used for bilirubin counts in your report as every country uses its own measurement system.

I hope this helps.

I am going to visit a doctor tmrw, will have your advice in mind. I'm reassured by your opinion, but still quite unsettled by feeling so rough for so long - over two weeks now (although it does seem to fluctuate).

The bilirubin count is marked as: um01/1 or mg/1 for the second reading.

If its 7.11 mg/L then its absolutely fine.
I can understand your concern. especially the nonspecific feeling of not feeling well and rough is terrible as you cannot do your work, but hopefully with the further test and your doctors meeting we will find out the cause of your problem and treat it as fast as we can.

The blood tests these days are all computerized everywhere but the gold standard still remains to identify the cells by a pathologist eyes, as no machine can identify the morphology of cells that a human mind can. so my recommendation would be a peripheral smear view and a repeat blood test in few days time.
Also, the stool test just to rule out any parasite infection in bowels as that can give you stomach ache and that mild uneasy feeling that you have been experiencing.

Hi Dr Parth

Since I spoke to you last, I have been to see a doctor here in Beirut He did a basic check up BP, lungs etc, which he said was absolutely fine and he thinks I may well have a parasitic infection of my intestine which is causing the symptoms.

Since I saw him, I have noticed that I definitely do seem to have pinworms, at least.

He has prescribed Vermox and a course of Flagyl antibiotics 500 mg for 7 days. I have taken the Verox and am two days into the antibiotics. I felt a little better this morning but really feel the same actually, with the same symptoms.

Two questions really-

Does a parasitic infection sound like it could cause that feeling of 'spaciness' and general malaise feeling I've been experiencing.

Would you be able to advise how long does it take before any effect from the antibiotic?

Hi XXXXXXX thank you for your feedback.

If you could see some tiny white worms then they are most definately pin worms and a potential cause of your illness and eosinophilia.
Vermox and flagyl in combination covers most of the parasitic infection and it is a good combination to have when you are not sure which is the exact species of parasite.

Yes, parasitic infections do cause bloating and spaciness feeling and also malaise to go with it. Since it is a subjective feeling not every patients experience it but i have seen many who do feel it that way.

1)Generally even 1-3 days of this drug therapy can cure parasitic infections. Remember these medications can also cause some uneasiness and vomiting so take a domperidone like antiemetic if needed.

2)If you still see worms after the dosing, a stool test and a repeat shot of these antibiotics may work after 3-4 weeks.

3)Also please repeat the blood count at the end of 4 weeks to confirm the normalization of eosinophil count.

4) After complete bowel cleansing by these antibiotics, if you still feel the spaciness and malaise say after 2-3 weeks then we need to search for other causes like nutritional deficiency, fibromyalgia and so on, but not right now. Mostly you will start feeling better as the parasites and medications both wane off from your body.

I hope that you will soon start feeling much better. The antibiotic combination is heavy and may cause some discomfort in form of nausea, bad taste and uneasiness but they would be helpful.

Also, if you drink alcohol, it is advisable to avoid for few days with flagyl in your system just as a precautionary measure.

Also please do repeat a blood count after a month or so just to be sure your eosinophils are back to their normal range.
