Question: Urine leakage and sudden frequent urge to urinate.
What are the tests to be taken for checking urinary incontinence or bladder incontinence.

I have gone through your query with diligence and would like you to know that I am here to help. I would like to start by briefly listing the causes for your reference, they are:

+Urge incontinence: Wherein there is an increased urge to urinate, and is usually due to conditions like diabetes, infections, etc.

+Functional incontinence: Is when some other underlying condition prevents you from reaching the toilet and emptying your bladder in time

+Stress incontinence: Is when there is pressure on the bladder which can happen when you lift a heavy object, sneeze, cough, etc.

+Overflow incontinence: Is when your bladder does not empty adequately enough, usually associated with an enlarged prostate (more common in men above the age of 50 years)

Now that we have discussed that, as you have also mentioned an increased urge to urinate, we should suspect urge incontinence in your case which could be associated with causes like infections, diabetes, etc.

As you have already conducted a urine routine examination, please do share the report with me so I can let you know whether there is a probable infection that is causing your symptoms.

1. Any family history of diabetes

2. Please also get your diabetic profile done (it includes RBS, FBS, PPBS and HbA1c).

There is a slight hint of (a possibility of) diabetes from those tests or pre-diabetes. So please do go about a diabetic profile which includes (FBS, PPBS, and HbA1c) and wrote back to me with the test results and we shall take it from there.

In the meantime, please also answer all the questions I have asked in my first query, that information will also help greatly.

2. Please also get your diabetic profile done (it includes RBS, FBS, PPBS and HbA1c). -- I will try to get this done as soon as possible.

3. Do you have any burning sensation while urinating? -- No

4. Do you feel excessively thirsty of late? -- No

5. Do you feel an increase in hunger? -- No, but due to my work timings messing with daily routine causing missing break fast often, or late lunch and very late dinner, eat more than usual

I am experiencing the pressure on bladder due to weights, while sneezing or coughing etc from last two years but it was never troublesome nor forced me to urinate.

+Urge incontinence: Wherein there is an increased urge to urinate, and is usually due to conditions like diabetes, infections, etc.

I got this urge to urinate frequently just from last 2 months but it increased in last 10 days.

Once I urinate or release gas I feel very normal like there is no issue at all for an hour or two before all starts again.

Haemoglobin A1C report will be available by tomorrow.

Hello once again sir.

Thank you for the tests and reports, all seems fine from an infection and diabetic point of view. But your blood sugar does seem to be slightly on the higher side, so diabetes cannot be ruled out yet, though you have said no to most of the common symptoms of diabetes, so it is still a grey area.

I would suggest a check on your gastrointestinal system as well, because you report an improvement after passing gas or even emptying your bowels so it could be that your gastrointestinal tract must be getting irritated and putting pressure on your bladder which is causing stress incontinence. In that case, finding the exact cause for your gastrointestinal troubles and treating it should relieve your urinary symptoms as well.

If you also concur, then please write to me with any gastric symptoms you have been experiencing off late, and we shall try to either find the cause or rule that out sir.

Thanks Shoaib for your quick reply.
I too think its less chance due to diabetes.

I used to drink coffee like 4-6 times but stopped coffee completely from last week and I did not experience stomach bloating, flatulence, gas, stomach heaviness, restlessness as much as I used to until last week.

I adjusted my timings also sleeping well, eating on time, avoided spicy food, soda, fried foods etc this whole week and felt lot better.

I am also using Nexium ( esomeprazole magnesium/ acid reducer ) capsules from last 3 days which treats frequent heart burn.

About the urine leakage and frequent urge, it did not stop yet.
I observed like after eating food or drinking water I am feeling it more compared to when my stomach is empty. I stopped regular exercise. I would like to check how it will be a couple of more days and take the next steps.

Please advise.

Hello once again.

I am liking your approach, and your signs of relief do point towards a possible chance of this brings a gastrointestinal problem, but sadly no relief to the urinary incontinence yet.

In that case, an ultrasound of your abdomen and pelvis would also provide us with some information a and probably even rule out a few conditions. A check on your bladder with pre- and post-vidal residue may also she'd some light on to the incontinence issue.

Do try for another week or 2 and take the esmoprazole in the morning before breakfast everyday and let's see how it goes. Hoping you find relief at the earliest, and please know that I am always here to help.
