What Causes Redness In Eye And Fever While On Gallbladder Medication?

default
Posted on Tue, 18 Nov 2014
Question: Hello Dr. Recently my Dr gave me some antibiotic for my gallbladder and since then things got worse ...was boiling up wasn't sure what was going on but I started on cactus juice and seems like i got some relieve from it but still off an on will run some light fever and some redness in one eye. What do you suggest? My Dr. thinks I have Autoimmune Hepatitis bit he is not sure about it and trying to put me on steroids and immune suppressant which is giving me a lot od side effects I cannot take that med?

Dr. also a biopsy was done on me and an MRI without contrast wasn't sure if with contrast would further damage my liver?? But I didn't get the with contrast done.

I need to know what are my other options in getting my high liver enzymes lowered and my immune system back to normal if an Autoimmune exists without
dangerous meds like steroids out there. I do have inflammation of some kind in my liver. I did try juicing plants my billiruben was going down and it did get to 1.5 but all the other numbers were still high but I was feeling but better ...question for you, does the other number still matter? I really don't want to take those meds cause of it's side effects it's got and besides I don't thinks it's suiting very well on me.

I forgot to mention that I do have some yellowing in my mouth and there was some yellowing on my face which seems to be going away gradually but off n on I do feel some slight fever ...any suggestions??

Brief Answer:
Need complete LFT reports

Detailed Answer:
Hello there.

I am Dr.Senduran,General physician hope to answer your query..

I would like to have more information to provide a better opinion

1)Furnish your liver function report details(SGOT,SGPT etc)
2)Are you still on antibiotic or any medications other than cactus juice ?
3)Any relevant history like joint pain, dry mouth/eyes or skin manifestation other than on and off fever, redness in eye, yellowness ?
4)Any history of treatment with steroids ?

However with the limited information would give my opinion as follows.

1)Redness in eye and mild fever ---> might be just a post antibiotic effect..Which antibiotic were you on and for which gallbladder issue exactly ? Needs evaluation which is already going on..

2)Steroids and other immunosuppressants need to be given if its an autoimmune cause..In your case, treatment may not need steroid or at least, high dosage is not needed.Inspite of high LFT values, you feel symptomatically better.Hence the numbers is not of great significance.

3)Exclusion of other chronic liver disease like viral hepatitis, drug induced liver injury etc, finding certain markers of autoimmune disease like serum gamma globulin level,ANA etc and biopsy interpretation would help in confirming the diagnosis

4)Gradual fading away of yellowness in your mouth, face may well correlate with decreasing tendency of bilirubin level..

Hope this information helps you ..Get back with the information needed especially your LFT values.

Awaiting your reply..

Regards

I was on Levaquin 500mg, take 1 tablet once a day for 5 days. It was for gallbladder infection.

I am not on any kind of medication at this time ..just on the cactus juice..it's main purpose is to remove any kind of inflammation, detox and remove old damaged cells and create new ones.
This is a report from last 2 weeks..I just got some blood work done yesterday waiting for the results will send it to you once I get it!!

3)Any relevant history like joint pain, dry mouth/eyes or skin manifestation other than on and off fever, redness in eye, yellowness ?

I was having severe dry mouth and eyes with Prednisone/ itchiness these days off n on. feel heat under my skin around my arms and neck area. Noticed brown small pots on my arms and stamoach area when I was on the infection med and the prednisone.

4)Any history of treatment with steroids ?

I was on steriods for only 3 days not anymore.

I was on cactus juice before and I did notice my numbers drop at one point and then I had stopped using it for a while when I started juicing but my number at that time went bk up but the billiruben went down to 1.5 rest were still high.

Dr.

I am very concerned about immune suppressant meds this may cause me to get cancers and other illnesses?

And how do u know for sure if I have autoimmune hepatitis?

Please advice, thanks
doctor
Answered by Dr. Senduran Narayanasamy (2 hours later)
Brief Answer:
Seems to be a autoimmune condition

Detailed Answer:
Gone through the reports carefully

Needs further evaluation.

Seems to be a autoimmune related liver and/or bile tract pathology.

The following values make me think of an autoimmune related condition
1)Gliadin antibody positivity --->A marker of other common autoimmune disorder called celiac disease.
(Note presence of any auto antibody -->increases the risk of another autoimmune disease)
2)Serum globulin -- 3.8(but near normal)--Could be a raising titre.

Increased liver enzymes(ALT,AST),ammonia and alpha fetoprotein --->Suggesting liver disease..in this case, high chances for autoimmune hepatitis..

Increased alkaline phosphatase(4 times the normal value),itchiness, liver enzymes(ALT,AST) less than 300 IU,near normal globulin level
--->suggesting bile tract inflammation/fibrosis(cholangitis)

If i am your physician, would advice you to test for also

1)Hepatitis B and C serology test
2)Fraction of gamma globulin(Serum IgG level)
3)Anti nuclear antibody(ANA),Anti-smooth muscle antibody(ASMA) and also anti-LKM(Anti Liver kidney molecule) antibody -->commonly positive in patients of autoimmune hepatitis..I would recommend this one if you have not done yet.

You can also discuss with your treating physician and decide..

Liver biopsy and MRI reports would help narrow down the diagnosis..

However in both the cases especially if its autoimmune hepatitis -->You would need atleast a low dose steroid or other immunosuppressant drug. With less dose and safer steroid like budesonide, side effects are less common and are tolerable.

But first need to confirm the actual diagnosis..Probably the reports of the tests done yesterday, tests that i have told, biopsy,MRI etc could help better to say whether you have autoimmune hepatitis..

Hope This helps you..

Regards

Hello Dr,

Just by looking at the reports what do you think do I have to be on these medications even if my numbers are going down with the juice?

Brief Answer:
No

Detailed Answer:
Hello XXXX

I didn't mean that you should be on medications..

As I said, the LFT numbers are not such high to be concerned at present. No need for steroids or immunosuppresants right away..

You need to be on immunosuppressive therapy only if your health is deteriorating I.e getting symptoms of liver disease like jaundice, itching, abdominal pain etc and LFT numbers keep on increasing (say above 1000).

Otherwise When you are symptomatically feeling better and LFT numbers are within acceptable limit, there is no need for steroids or other immunosuppressive drugs absolutely..

Hope you got the point.

Take care.

Question for you!!

1)Hepatitis B and C serology test

Q Is this the test for hepetitis A, B, OR C? If it is... I don't have none of those.

2)Fraction of gamma globulin(Serum IgG level)

A) I just checked previous report: Gamma Globulin is at 1.3

3)Anti nuclear antibody(ANA), not done

Anti-smooth muscle antibody(ASMA) not done

and also anti-LKM(Anti Liver kidney molecule) antibody -->commonly positive in patients of autoimmune hepatitis..I would recommend this one if you have not done yet.- not done

Brief Answer:
Find the answers below

Detailed Answer:
Hai XXXX

1)What do you mean ? Haven't done Hepatitis B,C tests yet or you have already checked and found negative ? ------no need for this tests if already checked and found negative within last 6 months.
2) IgG --1.3,When was it done? while were you on steroid? Level seems to be very low -----May be a relative decrease in early phase of disease but this not significant of autoimmune disease. Need repeat testing.
3)I would recommend to do the tests for these markers like ANA, ASMA, and Anti-LKM antibodies if feasible for you.

If you don't need further testing, get back with your recent reports, biopsy, MRI findings etc...I ll try to give my opinion regarding the diagnosis..

Regards

Thank you for your quick response!!

Yes, I have done the test for Hepatitis within the last 6 months for A, B, C AND E and all are negative.

The Gamma was done right before the biopsy in July 2014.

The rest still haven't been done. Will get those done soon maybe by this Friday. I have requested for my MRI, blood & Biopsy results... should have it within two or three days.

Hello Dr.

Just a quick question for you about the autoimmune test you requested. I found details about the kind of test that are done here for it..I just want to check with you and see if your request for the test what you asked for would be an addition to this as follows;

The following tests are used to diagnose an autoimmune disease:

antinuclear antibody tests—a type of autoantibody test that looks for antinuclear antibodies, which attack the nuclei of cells in your body
autoantibody tests—any of several tests that look for specific antibodies to your own tissues
complete blood count (CBC)—measures the numbers of red and white cells in your blood. When your immune system is actively fighting something, these numbers will vary from the norm
C-reactive protein (CRP)—elevated CRP is an indication of inflammation throughout your body
erythrocyte sedimentation rate (ESR)—this test indirectly measures how much inflammation is in your body

Please let me know, thanks!!

Includes tests which i requested.

Detailed Answer:
Hello XXXX

Hope you are well..

The antinuclear antibody tests and auto antibody tests to specific own tissues should include the tests which i requested like antinuclear antibody(ANA),anti-smooth muscle antibody(ASMA),anti-liver kidney microsomal antibody(Anti-LKM).Otherwise ask for the tests specifically and get it done.

Brief Answer:
It is OK

Detailed Answer:
Hello XXXX

No need to wait. Presence of jaundice won't affect the test results (ASMA,Anti-LKM,ANA) that I requested.Testing now would be appropriate to better find the diagnosis.
