Question: Hi Sir,
I am single, and when I masturbate, I donot feel any orgasm or pleasure while ejaculating. Even I don't sense when it is oozing out. I had undergone the varicocele surgery, now though I donot have varicocele, and though the sperm count increased from 20million to 60million, this symptom still persists.

Please find my observations below.

1. Orgasm is the feeling of pleasure during the height of excitation. It is not affected by doing the varicocele surgery. The medicines you are taking are not likely to influence getting an orgasm.

2. Most of the time inability to have orgasm in males has a psychological reason, it can be due to more focus on sexual need and the thoughts of having satisfying sex. This can be controlled by having a long and slow foreplay.

3. In masturbation, orgasm failure usually results from masturbating too fast or doing it roughly. Due to these reasons, there can be inability to have orgasm during masturbation.

It is mot clear why you are taking Tryptomer and for how long? If you have any stress related condition then that can cause inability to orgasm.

Sir,

a) I masturbate slowly. I never do it rough/hard.

b) Doctor prescribed me those medicines including Tryptomer for 2month,for treating the bladder incontinence,prostatis,urge to control urine.
I was also given Forcan150 for curing urinary tract infection.

c) Before varicocele surgery i.e when there is gross varicocele , I was not knowing when sperm ejaculates,until I see it came out.
After few days of varicocele surgery , only once I felt orgasm/felt the sense when sperm is oozing out /passing through penis.

From then, I never felt orgasm when it flows through penis, I only realize that sperm cameout.

Bottom line is ,when I was healthy before varicocele developed, I use to have satisfactory urge to masturbate & orgasm while ejaculating.
When varicocele started developing and after varicocle surgery I am deprived of that orgasm.
Then I don't know if there is any infection in testis, but a cyst was discovered on 2D Doppler test, for which antibiotics medicines are given.

d) As varicocele dilutes the sperm, does this also cause this symptom.
e) Do I need to consult sexologist or urologist for this.

1. If you have experienced orgasm before and are have problem with it now then it is probably a temporary condition and will get solved with medical consultation.

2. You has orgasm after varicocele surgery so that is also not a reason for your inability to have orgasm.

3. Rarely tryptomer can cause side effects which includes lack of orgasm and this needs to be investigated after having discussion with your urologist.

4. The cyst in testis is not a significant finding and you can ignore it.

5. Varicocele happens due to pooling of blood in the testis and it does not dilute sperms. The problem is varicocele causes less sperm formation and this has been treated in your case.

Sir,
So my issue can be solved by medication, I am confused whether to consult my Urologist who is treating for my Bladder incontinence or do I need to consult a sexologist for my case of "not having orgasm".?

1. In case your inability to have orgasm started after taking treatment for bladder incontinence and infection then you should discuss this problem with the urologist in detail. It is important to know if tryptomer can be causing your problem and this is temporary. Once treatment is complete then your orgasm will become normal.

2. If the orgasms do not return within few months after treatment is completed then it is probably a psychological problem and requires help from the sexologist.
