Brief Answer:
You need regular supervised treatment.

Detailed Answer:
Hello,
Thanks for your query.
I appreciate your efforts for medical consultation in so much distress.

Important aspects of your query are:
• 39 year old male
• Known case of Mood disorder for 12 years
• On treatment with medicines in Past: mirtaz, divalpro, tancodep, lamez, nexito forte, oleanz plus, pexidep cr, eslizen, tryptomer, ventab dxt, ventab xl, qutan, Soltus, prodep, vilano and Valium.

Mr XXXXXXX symptoms like poor concentration, poor sleep, anger, negative thinking, social anxiety and sexual problem, all are commonly present in depression.

You had already received several good medicines which are used for several purposes like: anxiety, depression, mood stabilization and insomnia. Medicines like nexito, paxidep, prodep and tryptomer also help in Premature Ejaculation.

In cases of depression it is important to rule out Manic symptoms or Mixed episodes, which is common in mood disorder. You have already received Divalpro, which is a mood stabilizer.

So, in case of mood disorder treatment is highly individualized. It depends on symptoms profile of patient, change in symptoms profile, treatment response, side effects of medicines, comorbid medical issues and inperson evaluation of patient on frequent interval with family members feedback. Due to that treatment require modification time to time depending on patient condition. As your problem is old (since 2005) then you require regular long term treatment.

It is better if you can meet personally for treatment (If you live in XXXXXXX NCR). If you live far, then meet nearby psychiatrist.

Apart from that following lifestyle changes will help:
-Fix sleep and wake up time, No day time sleep
-Avoid excessive caffeine (tea/coffee/cold drinks)
-Avoid any substance use.
-Switch off TV/Mobile 30min before sleep
-Start daily Morning brisk walk for 30 minutes.

I hope this information has been both informative and helpful for you.
Wish you good health.
Regards,
