Brief Answer:
likely PCOS + HYPERPROLACTINEMIA

Detailed Answer:
Hello,
Thanks for the query to hcm,

With high prolactin levels and irregular cycles with weight gain indicates that you may be suffering from PCOS and hyperprolactinemia and increased insulin resistance.
You need to undergo an ultrasound pelvis to look for polycystic ovaries and a serum testosterone level to estimate high androgen levels.
If PCOS detected then you would have to be placed on low dose oc pills for at least 6 months to regulate hormone levels and also 3 week course of cabergoline 0.25mg twice a week for decreasing prolactin levels.
Thirdly weight has to be reduced by regular exercise in order to decrease the insulin resistance.
you have to be consistent. Hence it is important that you visit your physician soon and get evaluated completely to start the treatment soon.

Hello,
You have to understand that its the hormones which are the case of your irregular menses and also the weight gain is hormone related .
It is important the you should be placed on cabergoline sooner and also on low dose oral contraceptive pills after a withdrawal bleed with progestins and then follow a regular exercise and diet schedule to return back to regularity of cycles, decrease prolactin and decreased weight status.
Acne shall also improve once androgen levels in the body reduce.
Please get evaluated for POLYCYSTIC OVARIAN SYNDROME and rule it out.
