What Causes Gnawing Burning Sensation In Abdomen, Nausea And Intermittent Constipation?

Question: I have been experiencing a gnawing and burning sensation in my upper abdomen for 2 and 1/2 months now. Sometimes mild nausea and on and off constipation. My heart pulse has slowly risen higher and higher over the lasts months, and usually my resting pulse is now between 88 and 112. My pupils pulsate back and forth in bright light, and wont constrict for more than a second. Allergies have gotten worse and my throat seems tight. I completely lost my appetite for a week, but it has slowly come back. I often feel very cold and get chills often. My muscles sometimes ache and twitch mildly. Could this be a stomach issue or a hormonal issue? Or both? What could cause this?

I read your concerns and understood your concerns.

Your symptoms may most probably are related to gastric problems (gastritis). However they may be a side effect of oral contraceptives.

This is why I recommend to stop oral contraceptives to see if the problem persist. Otherwise other examinations are needed to find out the exact cause of your problems.

If you were my patient I would recommend to do the following examinations:
- Abdominal ultrasound
- Upper endoscopy
- Stool test
- Hormonal tests (especially thyroid hormones)

Thank you for your answer. I have some further questions.

-Is the stomach connected to the sinuses as well as the throat? As I have been experiencing worsening allergies and pressure on throat and sinuses, would stomach problems cause this?

-The same question about the heart rate, can stomach problems increase your resting heart rate for a longer period of time, as I have had a higher heart rate for the last two months, shortly after my stomach problems started. Also, can the stomach problems cause the trembling of my hands?

-Should I be concerned? I will have to wait at least 3 weeks until i can go to the doctor again because of traveling and christmas vacation. Since i function quite normally, is there a need for fast medical help?

-What can I do to make things better before I see a doctor?

-Could all this be caused by stress only? The symptoms started after moving to a new country to study three months ago. Although I have not been feeling stressed until just recently, is it possible that my body feels stressed because of the new surroundings and acts out on it without my mind noticing?

-I used to be quite active, but the last 3 months I have been sitting still a lot and spent a lot of time in my bed, could this cause my symptoms?

- When reflux is present, the reduced pH of the stomach will cause persistent inflammation of the esophagus and throat. This is because the stomach liquid is turning back to the throat and will cause continuous inflammation.

- Yes, when stomach is distended would cause pressure to the heart which is experienced as heart rhythm alterations.

With regards to shaking hands, it might be due to reduction in:
- blood pressure
- blood sugar levels.

I advise to measure your blood pressure and sugar levels at the moment you are feeling "shaking hands".

- There is nothing worrisome and emergency. You can discuss with your doctor to start therapy with:
- PPIs
- ranitidine

- With regards to home remedies, I highly advise to:
- avoid foods/drinks that will aggravate your symptoms
- take OTC ranitidine and antacids
- do not lay down immediately after eating
- try to increase physical activity (e.g. walking).

- Your symptoms might have been triggered by changing town as you have been more under stress and have changed diet.

- Being more active will ameliorate your symptoms. Lack of physical activity might also be another cause.
