Brief Answer:
Could be due to Enlarged prosate associated with skin infection

Detailed Answer:
Hello
Thanks for query .

Frequent urination with hesitancy and poor flow that you have is due to Enlarged Prostate which is very common at this age.

Irritation around tip of the penis is mostly due to infection of urethra and rash with itching that you have is due to fungal infection of skin .

Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.

Further treatment will depend upon result of these tests and final diagnosis.

In the mean while start taking Norfoxacin and Nitrofurantoin twice daily along with Tomsulosin once daily

For skin infection over buttocks use topical anti fungal ointment Meconazole twice daily .
