Brief Answer:
Take tablet Pantoprazole 40mg with Tab.Gas-X

Detailed Answer:
Hi,
Welcome to health care magic,

I have read your query and understand the concern.

Feeling boated in stomach or discomfort or fullness in stomach could be related to dyspepsia, constipation or acute gastritis.

But it needs further evaluation by clinical examination and various tests to arrive at a certain conclusion if problem persists longer.

However,at present you should take symptomatic over the counter medications and dietary precautions to get relieved from this gas pain and bloating stomach.

1) Take over the counter antacids tablets like Tab.Pantoprazole 40mg with Tab.Gas-X twice daily to get relieved from the symptoms of bloating and sharp burning gas pain.It may give immediate relief within few hours.

There is NO need to take cyclopam or buscopan because this pain or discomfort is more likely related to stomahc gas and bloating feeling.

Cyclopam (contains dicyclomine-an antispasmodic and paracetamol-a NSAID pain killer) may further cause gastritis and may cause more discomfort and gas pain.

2) Take syrup digene 2 TSF three times a day will also be helpful.

3) Avoid hot, oily and spicy foods.

5) Avoid eating full stomach one at a time rather, take small meals frequently.

6) Have soft diet only for initial few days.

7) Avoid complex carbohydrates like beans, potato and carbonated beverages and milk based products which are hard to digest.

8) Drink plenty of water and fruit juices the day.

9) There are certain digestive enzymes (Unienzyme) available over the counter which will also be helpful in the digestion of carbohydrates and minimize the gas production. You may also continue with psyllum for motion as you are taking.

We may expect to see improvement from these symptomatic over the counter treatment, but in case if problem persists longer, then i will advise to see physician or gastro-physician who can order various tests (like routine blood works, abdominal ultrasound) and can come to a particular diagnosis and start the treatment.

Hope this will be helpful.

Thank you for your advice. Regrettaby the pain and heaviness above the belly button continues.
Presently not in a position to get a clinical check up. Pain is dull and colicky. Most troublesome is the heaviness. It is now 2,days and commenced after a rather spicy meal ordered in.Since then,
ate light, took Pantocid 40 about 30 mins before meal. Took Cremalax 20 mg at night for motion.
Took Drotin at night for pain relief though you had not advised but the pain became bad.
Suspect I may have an infection. I have Skifi Oz readily available. But there is no fever.Pain is constant.I dont know whether to take Skifi Oz.Do advice.

Brief Answer:
skifi-oz can be tried.

Detailed Answer:
Hello,
Welcome again,

I am sorry to hear that you are still suffering from stomach ache even after trying symptomatic medications.

If pain is dull and colicky with bloating then the most likely possibility is acute gastritis which I suspected earlier.

However,we should first try with over the counter symptomatic medications and if they do not help much then appropriate antibiotic should be prescribed for a shorter course like 2 or 3 days.

Skifi-OZ is a good XXXXXXX spectrum antibiotic with having anaerobic coverage and if it is readily available then it can be tried twice daily, however you must keep in mind that it is a prescription based drug and should be taken under doctor's supervision.

So,apart from dietary restriction,

You can try skifi-oz with pantoprazole 40 mg,tab.gas-x twice daily if it available over the counter in nearby pharmacy.

Continue with cremalax for motion and if there is burning or acidity then take syrup digene which will also be helpful.

But I am not in the favor of taking any pain killer (NSAID) or anti-spasmodic drug (like cyclopam, buscopan or drotin) since the pain likely related to gastric inflammation either due to some infection or by oily/spicy foods. Also this pain killers have not given much relief since you have already tried them.

Also careful with your diet or I would recommend to take only fluids and fruit juices for initial couple of days and avoid routine/solid diet.

Hope this helps and let me know if you need more clarifications since I am online right now for couple of hours.

I will take only juices or dry toast for the time being. I just hope its not an ulcer though I have no such problem in the past. Pain is dull and colicky and stomach is bloated.

Brief Answer:
No need to worry about ulcer.

Detailed Answer:
Hello,
Thanks for the quick response.

I do not think there is any chance of ulcer here especially from this symptoms,so would advise you NOT to think about stomach ulcer.

You should continue with this treatment and strict dietary restrictions as we discussed.

However,if the problem persists even after 2 to 3 days then better to see some local physician who may order some routine blood works, abdominal ultrasound and upper GI endoscopy if needed.

Wishing good health.
