Question: Hi my name is XXXXXXX I am 39yrs I been experiencing sweating spells for the past few months and the last two days I have been having full feeling in my stomach and left chest pain and pain in my right shoulder I take meds for diabetes and thyroid and herd and depression please help me

I understand your concern, and would like to explain that there is nothing to worry about.

Your latest symptoms seem to be gastric related (possible gastroesophageal reflux, acute gastritis or dyspepsia). Do you have other symptoms like burping?

Have you tried taking any antiacids or PPIs (omeprazole, lansoprazole,etc). The relief of your symptoms by these drugs would be a strong argument in favor of this cause.

What about your blood pressure? You should also closely monitor your blood pressure. A high blood pressure would indicate anxiety or a thyroid hormone imbalance.

An orthostatic hypotension (a decrease in your blood pressure while standing up), could cause episodes of dizziness and sweating.

What is your actual therapy for depression and hypothyroidism?

A thyroid hormone imbalance (a higher dose of levothyroxine), could be related to your episodes of sweating spells.

Anxiety could also mimic this clinical situation. You should know that depression and anxiety (including panic attacks) are closely related. Have you tried any anxiolytics (librium, alprazolam or other benzodiazepines) during these episodes? Have they relieved your symptoms?

I would recommend consulting with your GP for a careful physical examination, a resting ECG, cardiac enzymes, a chest x ray study, an abdominal ultrasound and some blood lab tests:

- complete blood count
- thyroid hormone levels
- inflammation tests (PCR, sedimentation rate)
- fasting glucose
- kidney and liver function tests,
- amylaze plasma levels (possible pancreatitis)
- electrolytes plasma levels etc.

If suspicions are raised for a GERD (gastro-esophageal reflux,), esophageal spasms or a hiatal hernia, a GI endoscopy should be performed.

A cervico-thoracic spine x ray would be needed to examine better your back bone for possible disk displacement causing irradiating chest and arm pain.

A head up tilt test may be needed to rule in/out a possible orthostatic hypotension, if suspicions are raised during physical examination.

You should discuss with your attending physician on the above mentioned issues.

Meanwhile, I recommend you taking a PPIs or an antiacid which could relieve your gastric discomfort and chest pain.
