What Causes Dark Blood Spotting After Taking Depo Provera Shot?

Question: Hi I had just a shot of Depo Provera in Dec of 2012 after the birth of my son. my menstrual cycle resumed last month but since it resumed I've been spotting dark blood. My cycle came in march and it was very scanty. It stopped a few days again and I'm still spotting dark blood. Now should I be concerned? I also have acute pain that comes and goes on the upper right side of my stomach The dark bloody discharge is thick and ongoing. starts and stops and resumes. Is this a post Depo Provera or should I be concerned? What could be causing the pain in the upper right side of my stomach?

Brief Answer: Do not worry Detailed Answer: Hello and welcome to HCM, I understand that you had a depo shot in December 2012, almost 15 months from now. Your son must be 15 months old at least by now. I am not sure if you have been breast feeding him. The depo shot does not have any association with your current symptoms. It lasts for only 3 months after that it is gone from your system by four months at the most. So do not think about it now. Regarding your current complaints, menstrual pattern resumptions after delivery are variable. Some women may have an abrupt return to normal cycles, some have irregular cycles, some may have spotting and so on. Breast feeding has an impact on cycles as the hormone prolactin that is secreted during feeding tends to delay periods. However after a child is weaned, the frequency of feeding is decreased and may lead to irregular periods. Hence it will help to know if you are breast feeding. Other reasons for this may be hormonal imbalances, ovarian cysts [ you already have a history] and cervical polyps or fibroids [ non cancerous tumors of cervix and uterus. They can be detected by an examination and blood tests as well as ultrasound. Other cause may be pregnancy and missed abortion if you have not used contraception. A urine pregnancy test and ultrasound will help in diagnosis. The right upper side pain may be related to digestion problems or issues with gall bladder like gall stones [ quiet common] liver, or muscle pain. An ultrasound of the abdomen will help in diagnosis. As of now, my suggestion is that if you are breast feeding, stop it if possible. Get a urine pregnancy test done. If it is negative [ still a missed abortion cannot be ruled out completely] and you continue to spot and have pain, get an examination and testing from your gynecologist to rule out the cause. Meanwhile try regular exercise, good sleep and a healthy diet like fruits and vegetables with little helping of meat avoid fats and oils. Brief Answer: Thanks for the update Detailed Answer: Hello and welcome back. The stopping of the breast feeding may be responsible for resumption of periods. In this case, if there has been no contraceptive use, please rule out a pregnancy by urine test and an ultrasound for a missed abortion. If contraceptives have been used regularly, urine pregnancy test is not needed, just wait for a few days to see if it settles. If the pain and discharge continue it is better to get an examination and an ultrasound of the pelvis and abdomen to look for cause. Hope this was helpful. I am always here to help you.
