Suggest Ways To Regulate Blood Sugar Levels When Diagnosed With Diabetes

Question: I'm Diabetic for about 11 yrs now - have been consulting different doctors and hence had to change a lot of medicines.

Recently, One of the Doctors prescribed me Mixtard - Insulin Injection along with Tab Gemer 2 mg & Tab Jalra - 50 mg & Roseday - 10 mg

Within One month of the above medication - There was a visible improvement in my Blood Sugar & was in the permissible limits.

Subsequent to this I had a minor Stroke - deviation of angle of mouth to the right side - which is now cleared with medication and regular physio-therepy

For my Diabetes, the doctor had given a new Insulin - which is now 5 times a day - with about 20 units and has asked me to stop the earlier sugar medicine- this is not helping me bring down my Sugar levels - despite strict dieting and excercises.

My question now is :
Can I get back to my earlier medication - Mixtard - Insulin Injection along with Tab Gemer 2 mg & Tab Jalra - 50 mg & Roseday - 10 mg

Will the above medicine / Insulin - have an adverse effect - because of my recent - Minor stroke issue ?

Thanks for your question.
After going through your question I can understand your concern.

I want to answer your question directly-
1) Yes, you can get back to your earlier medicines Inj. Mixtard, Gemer 2 and Jalra 50 and Roseday 10mg but it requires appropriate blood sugar monitoring as suggested below and accordingly need changes in dose of insulin if required.
2) Above medicines usually don't have adverse impact on your recent stroke, unless there are no low blood sugar level (hypoglycemic) events or other factors like deranged kidney or heart function that prevent their use.

I would like to advise you to follow strict diet and exercise and to monitor blood sugar at home by using glucometer (SMBG- Self Monitoring of Blood Glucose) as follow-
- Fasting (Before breakfast)
- 2 Hours after breakfast.
- Before lunch.
- 2 hours after lunch.
- Before Dinner.
- 2 hours after dinner.
- Early morning 3am.
And not just Fasting and Postprandial as you are doing.
It is usually advisable to do as much as readings possible from all of above readings daily.
Otherwise at least 2 or 3 reading should be done daily.

If required additional readings should be done in case there are symptoms of low blood sugar (hypoglycemia), in that case you should also notice date and time of incidence any change in meal pattern, increased physical activity on that particular day.

Depending upon weekly chart your doctor can adjust dose of insulin.

Mine is a Job - which calls for lot of Travelling - atleast 4 - 5 days in a week - out of my Hometown - XXXXXXX - which primarily means - eating outside from the Hotel.

The Minor Stroke episode of mine is about a month old now - & as I explained in my earlier question - I'm following strict Diat / Exercises - about 45 mts. walking in the morning & working from Home - for the last 4 weeks.

I did check with the neurologist and got clearance for me to get back to work from next Monday onwards.

Can you please confirm If I'm okay to resume duties / travelling from next week & what precautions do I need to take for the current issue of huge variations in my BSL's ? Any Specific suggestions for my diet ?

After going through your question I can understand your concerns.

I will advise you-
1)     Definitely you can resume your duties and traveling.
2)     Precautions to be taken and dietary advise-
-     Regular blood sugar monitoring as advised previously.
-     As you have to travel most of the time as a part of your profession and there is outside eating, you should try to maintain timings of your meals and medicines, though it may be difficult.
-     There is increased risk of low blood sugar levels (hypoglycemia) in your case as a part of missed, delayed meals, variable duration in between two meals.
-      You should always carry some snack at least XXXXXXX biscuits and sugar tablets with you when ever there are symptoms of hypoglycemia like sweating, uneasiness, unusual hunger, light headedness try to check blood sugar level with glucometer if possible. If it is not possible to check blood sugar or glucometer reading is below 70 you should use sugar tablets and snack.
-     Also consult your diabetologist for the use of antidiabetic medicines and insulin that have lower chances hypoglycemia.
