Suggest Treatment For Vaginal Vault Infection

Question: How do they check for vaginal cuff tear?
I think I may have it but my GP did an internal exam but could not see very far inside due to my pain.
I was sent away with antibiotics and pain killers following a teleconference with my gynecologist.
I have never felt pain like this before. Do I need an ultrasound?

I'm 9 weeks post hysterectomy and it occurred during first time sex since op (not rough).

Hysterectomy also has a scar in the vault of vagina. Post hysterectomy, at times, if this scar gets infected of develops swelling in it, it causes problems like post-hysterectomy bleeding, low backache, foul smelling discharge, painful intercourse, etc..

This scar usually does not call for inspection unless some complications have developed. Since you had painful sexual intercourse, such internal examination was undertaken.

Post hysterectomy scar inspection is to be carried out by a Gynaecologist who has required instruments in his/her clinic. Such examination is done either by Cusco speculum or by Sim's speculum with Anterior vaginal wall retractor. The later pair of instruments allows good vision of complete vagina; however, this examination has to be carried on by taking the waist of the patient to the edge of the examination table with both the thighs flexed and abducted. Cusco speculum shows very limited field inside the vagina.

The very fact your vaginal examination was extremely painful indicates the possibility of some infection to the scar inside the vagina; hence you have been put on a course of antibiotics. After a completion of the course of antibiotics, please submit yourself to vaginal examination again; however not by GP but only by a Gynaecologist.

If the pain is unbearable, you may start some anti-inflammatory painkiller like NSAID which will give you temporary relief from the pain.

Till you are completely cured of the infection, please observe complete sexual abstinence. In fact, give further more 6 weeks period for proper complete healing of the scar in the vaginal vault.

Would an ultrasound assist in detection?

Dear XXXX,

If there is any collection of fluid or development of some tumour in the pelvic region, ultrasound could be of help. However, your picture suggests the lesion in the vagina itself. Please complete the course of antibiotics, get intravaginal thorough inspection done and then if still there is pain in the lower abdomen, you may think of submitting yourself for ultrasound. At this stage, I do not feel ultrasound is indicated in your case.
