I can understand your concern.
Pulmonary artery enlargement is due to Pulmonary hypertension (PH).
And in your case it is due to COPD.
Pulmonary hypertension is irreversible condition.
And hence Pulmonary artery enlargement is also an irreversible condition.
Proper control of COPD is needed to prevent further worsening of PH and hence Pulmonary artery enlargement.
So consult pulmonologist and get done proper control of COPD.

Sorry to say but COPD is irreversible condition. Damage made in the lungs can not be reverted back to normal.
But yes, we can prevent or reduce progression of the COPD.
All these depends on following things.
1. What is your last spirometry test?
2. Do you smoke?
3. How many times in a day, you are taking your inhalers?
Stem cell therapy is not having proven role in COPD. Lung transplantation has proven role in COPD.

I do have a pulmonolgist and take spiriva a symbicort with albeuteral prn. I am on 02 at 3l. At the moment I am on levequin and steroids as I picked up a pneumonia. I have been able to manage myself without hospitalization except for in late 2013 when I had a spontaneous pneumo and it was then that I started copd interventions. I am learning to cope but a bit stubborn about accepting I can not get better. The hype around stem cell makes it very tempting to try. But I can't seem to find anyone other than those selling it to give me much encouagement. Is it our usual scientific cautiousness that will delay it hitting mainstream? If it won't work why not? Why are we not progressing on curative treatments other than the mechanistic replace it approach ? I am implementing all the management I terventions such as diet exercise and hydration and have not smoked for almost 3 years. Just not good at accepting that things can't be improved if we will not accept the status quo and really try. Seems like the focus is always to throw a med at something to treat symptoms or cut it out and replace it. Any other ideas or suggestions? I can understand your concern. I can feel your frustration.
Most common cause of COPD is cigarette smoking. And incidence is much higher in female smokers. This is the reason why there is written warning on all cigarette packets. And idea and understanding of saying 'prevention is better than cure' lies true when it comes to COPD or other smoking related organ damage.
I am not criticising you or have bad feelings for you. But when you say that doctors are not progressing in curative treatment and give only drugs and replacement, I must say that we doctors did not tell you to smoke in the past. You yourself started smoking despite of warning on cigarette packets.
Anyways sorry for being harsh.
Other than smoking cessation and inhaled drugs following things should be done in COPD patients.
1. Adult Pulmonary vaccination
2. Pulmonary Rehabilitation.

Yes I have done vaccine and I am doing exercise with a trainer and use a spirometer to exercise my lungs

You know Dr. I have been a Nurse For over 30 years m y clinical expertise is Ed and trauma I have been a nurse leader for over 20 years and now have my own consulting company in the health and technology area

I tell you that only so you will know who you have just spoken down to. My questions or remarks were not directed at physicians or you specifically. I do not nor does any patient benefit from a pious lecture on smoking. My questions and concerns are legit and I accept responsibility for my illness how I got it and the management of it. As I reflect on your attitude and response. I guess I have the answer to the question. It seems the reason there doesn't seem to be any interest in other alternatives is because the patient with COPD has done it to themselves and deserves it. Very disapointed in your attitude and platitudes was hoping for some new insights.

I can understand your concern. I know I was a bit harsh and already said sorry for it.
Research work is going on for lung regeneration after smoking damage.
But believe me, nothing seems promising.
Newer drugs like indacaterol, glycopyrolium, roflumilast etc are available but they are not curative or revert the damage.
They are only to prevent further progression. They will not revert and normalize lungs.
So continue oxygen, pulmonary rehabilitation, smoking cessation and inhaled drugs.
