Suggest Treatment For Itchy Scab Above Anal Region

Question: Dear doctor i am a 34 year old female. For past 12 months i have had a itch which has turned into a shinny scab inbetween my bottom just above my anus.

Its very itchy but not always. It has gradually got bigger.

I have attached an picture for you to evaluate.

What could this be?

My Dear Madam,

Welcome to this portal with your query.
I am Dr V XXXXXXX a Sr Dermatologist.
I have examined the pic and taken note of all your concerns.

I will help you in recovering from the problem being faced by you.

The problem seems to have commenced as an intertrigo and on close inspection of the pic, I also found evidence of a fungal infection developing on one side.

Under the present situation, i would have taken a scrapings test to confirm presence of fungal infection. If it were positive, i would have suggested the following for fast recovery:

1 keeping the affected area dry and not allow misture to be retain for long by frequent wiping.

2 Use Eumosone M cream which is a combination of Clobetasone, a comparatively weak steroid to reduce inflammation and antifungal Miconazole initially for three days and followed by plain Antifungal like miconazole (OTC).

In place of Clobetasone one can use Hydrocortiosne which is available OTC

3 Terbinafine 250 mg twice a day for oral antifungal therapy

4 Zyrtec or a comparable anti allergic to relieve itching.

The medicines generally take about one week for complete clearance of infection. After that, one needs to maintain dryness and ensure that one is not having high blood sugar levels which will predispose a person to have repeat of the same problem.
