Question: My LMP 16.10.2014.My ultrasound shows on 26.10.2014 uterus acutely retroverted.multiple follicle in both ovaries.one developing follicle 1.63cm size in left ovary. polycystic looking ovaries.my age 29. Is there any chance to become pregnant in this month?how?

According to your follicular study, one developing follicle reaches 1.63 cm on D11. The maximum follicular diameter prior to ovulation is 2.1 +/- 0.35 cm with a wide range of 1.5 to 2.8 cm. In this cycle, you have quite chance of pregnancy.

Your ovulation time should be monitored properly and follow this guideline to maximize the chance of pregnancy:

1) If your period is quite REGULAR, you should involve in unprotected sex close to your ovulation day (14 days prior to next due date) or fertile days (12th to 18th day of each 28- 30 days regular cycle) for at least 6 to 12 months.

You can use ovulation prediction kit or LH kit to predict ovulation day in middle of the cycle. It is available in all medical store. Otherwise, you can proceed Serial Follicular Study (D11 to D16) in every cycle after consultation with your doctor.

2) You must take a healthy balanced diet with more fresh fruits and vegetables. Low carbohydrate diet is preferable. Take vitamin-E and Folic acid supplements for 3 months in preconception period and there after.

3) Do light to moderate exercise for physical fitness and weight control. Avoid mental stress completely by regular meditation or yoga. Maintain a healthy life style.

I take oralcon last in july.After that we start to trying. Normally I have regular cycle.bt last period was 14days late.After that the normal period has started.Then on D11 I do that usg which mentioned before.Why my LMP so late??what does mean multiple follicle and one developing follicle??

Thanks for follow up query.

1) You DON'T take any combined hormonal pill or contraceptive pill while you are trying to get pregnant. It should be stopped completely.

2) Progesterone pill can be taken from D16 to D25 to regularize period if you get irregular period in every month. In that case, you should consult with doctor.

3) Period may be delayed due to hormonal imbalance and mental stress (MOST common causes). Poly cystic ovary, over weight, high prolactin level, irregular diet habit, pelvic infection etc are some other causes of late period.

Take LOW carbohydrate diet and control body weight through physical exercise. Do meditation or yoga regularly to avoid stress. These are the KEY steps to get regular period.

4) It is quite NORMAL to find multiple follicle and one developing follicle prior to ovulation. Physiologically, ovarian follicles (from BOTH sides) starts to grow up and compete with each other for dominance.

One of them (from RIGHT or LEFT ovary) continues to grow and develop to release an ovum in the middle of the cycle by the help of LH surge. It is called DOMINANT or DEVELOPED follicle.

Usually, approximately 10 Graafian follicles begin to mature where one becomes a dominant follicle and the rest become atretic ovarian follicles.

As it is D11 of your cycle, follicular study reveals ONE developing follicle and multiple small follicle. After ovulation, all of them will regress.

I upload the latest usg report.will u analyze the each point please?do I have polycystic ovary?without kit how can I understand the time of ovulation?I never have any egg white discharge.how do i confirm whether i am ovulate or not??is there any chance to be not getting pregnant with the uploaded usg report??it says polycystic looking ovaries though i have multiple follicles including one developing follicle measures 1.63cm.how much time to be taken to release an egg from this follicle??when should i do intercourse in between for this month to become pregnant???

Thanks for additional queries.

1) I have already explained your test reports. Just go through my previous text. At this stage (D11), multiple follicles with one developing follicle are NORMAL thing as you are approaching towards ovulation.

As you have gotten your period regularly, then you wouldn't be worried about that. They may be functional cyst (NORMAL) and quite harmless.

2) You should use Ovulation Prediction test kit or LH kit in the middle of cycle to monitor your ovulation. It is the BEST method and based on LH hormone, which precedes ovulation.

Other methods: a) BASAL BODY TEMP - it changes during menstrual cycle. Progesterone released during the menstrual cycle, causes an abrupt increase in BBT by 0.5°C at the time of ovulation. It enables the fertile window through the use of commercial thermometers. For that, you have to measure body temperature each morning for next 10 days to track the changes.

b) CALENDAR METHOD: Ovulation occurs approx 14 days prior to next due date and you have to track down in every month. It is quite useful for regular cycle.

3) Your fertile days will be D12 to D18 of each 28 to 30 days REGULAR cycle. Therefore, you have to involve in unprotected sex regularly during these days to enhance your chance of pregnancy.

1) It is expected that DOMINANT or DEVELOPED follicle will release ovum within 1 to 5 days (from D12). After ovulation, ovum will survive 24 hrs inside fallopian tube to be fertilized. During this time, you have to involve in unprotected sex regularly. You should use ovulation prediction kit also to track ovulation.

2) Multiple small cysts (may be Functional or Follicular) appear on the ovary during scanning. It is usually called Poly cystic looking ovary.
