Suggest Treatment For Anxiety

Question: Hello Doctor,
I am XXXX, 36 years old, Diabetic and Hypertensive. I had been taking a medicine called Deanxit for about 7 years to manage anxiety ans stress as suggested by my family physician. Last year, Deanxit was banned in XXXXXXX since then I have been facing the problem of stress and acidity which is why I was prescribed Deanixt earlier after which it was fairly controlled. I was still able to manage without Deanxit for a period of 6 months after which I started taking Etizola 0.5 mg when I had acute symptoms of stress or panic but not to any benefit, as it causes mood swings the next few days and mild hissing sound in the ears which is getting frequent. Is there any other medication apart from the ones mentioned above to control stress and anxiety, as I often tend to feel any issue more than what it really is, but this feeling subsides fairly upon use of Etizola 0.5 mg for about 5-6 hrs and the symptoms tend to come back again. Please advice.

Thanks for writing to us. You have very well narrated your problem.

Your history suggests you are suffering from some type of anxiety disorder it may be generalized anxiety disorder or panic disorder or some other anxiety disorder.
To diagnose the problem detail history and mental status examination are done by a psychiatrist.
Deanxit was wrongly prescribed by many physicians so it was banned. Patient becomes dependent on it and it actually does not treat the problem from root.
Etizola is good anti anxiety drug but it works almost as symptomatic treatment.

Specific treatment of anxiety disorder includes both medicines and cognitive behavior therapy(CBT).

In mild cases only CBT is sufficient. In moderate to severe cases initially medicines and then CBT is added. And later on medicines stopped gradually.
If medicines alone are used chances of relapse are more after stopping the medicines.

In medicines SSRI group of drugs(fluoxetine, paroxetine, sertraline, escitalopram) are main stay of the treatment.I prefer paroxetine. Benzodiazepines group of drugs(clonazeapm, lorazepam or etizolam) are used initially for short duration for fast symptomatic relief.

This specific condition is treated by psychiatrist. Medicines I mentioned are prescription medicines. You need to consult doctor for the same. Ir request you to consult psychiatrist personally to get rid of your problem.

Hello Sir,
Thanks for the response, I have used clonazeapm (Petril) twice (2 courses of 3 days - 3 tablets) and always get a delayed reaction of allergy (once the allergy was Anaphylactic) and my physician asked me to stop it and then prescribed Deanxit. I have also used Sertraline (Serlift 25) prescribed by my physician for 3 days and found it highly uncomfortable especially while sleeping. With Serlift 25, which I used for 3 days, I found my problem to be getting worse and just stopped it after 3 days. I was told to manage with Restyl (0.5mg) and etizola on a need basis and not regularly. I have also been given a prescription called Tryptomer 10 by my physician which I need to take. He told me that tryptomer works in the same way as Deanxit. However, I fear any adverse affects it may have and have not taken it.

Please advice if I can go ahead and take only Tryptomer 10 and discontinue Etizola 0.5.

Etizola on needed basis is ok for anxiety but till when? Ultimately you have to come out from this problem. I do not recommend restyl(alprazolam) as it can cause severe dependence.

Anaphylaxis with clonazepam is probably the rarest thing. I have never seen yet. And etizolam is same group of drug like clonazepam so you should be allergic to etizola in that case.

Tryptomer and deanxit are totally different. Trytomer(amitryptiline) is tricyclic anti depressant. Deanxit contains flupenthixol(a typical anti psychotic) and Melitracin(an antidepressant). Mechanism is totally different.

Sertraline may not be tolerated by everyone. Sometime SSRI cause problem initially but after initial period it does not cause any problem. SSRI will take time of 2-3 weeks for showing improvement. 3 days are not sufficient for effectiveness.

I do not advice to continue tryptomer in this way. 10 mg is very low dose, you need higher dose which can cause side effects like dry mouth, constipation, postural hypotension.And it is very rarely used for anxiety problem nowadays.

Actually your problem is psychiatric and you are treated by physician. Believe me you can be treated. You need help of professional who can treat such type of problem. Psychiatrist is the person who can help.For right treatment proper diagnosis is necessary which is lacking in your case.
