Suggest Treatment For Abdominal Pain

Posted on Mon, 6 Apr 2015
Question: Hi, I have had pain in the middle to down left part of my stomach for about 2 years now. It seems to come and go, and I couldn't find any pattern to it. After a while I noticed that the pain usually occurs after I eat. It seems to be getting worse. I can be completely okay for couple days or with barely noticeable pain. But then there are days like today. I just had a meal, and now I am in such a pain that I cannot walk. I believe it will pass as it always does about an hour or so. But haven't had such a pain like now yet. Can you please advise what it can be? Thank you for your help.
Kind Regards, XXXXXXX

Brief Answer:
X-ray and Barium study

Detailed Answer:
Hi.
Thanks for your query and an elucidate history.
I have read and understood your history. Full Gynecological examination and trial with Normocol noted and now waiting for an appointment with Gastro.

To recapitulate: Female / 34 - recurrent suffering for 2 years - no pattern - pain in the middle to down left part of stomach - occurs after you eat - usually passes in an hour or so - today it is severe enough not to allow walking - can be OK for a couple of days - wants to know the reason.

May I please know a few more points to add to my knowledge:
- Is there associated bloating or distension of the whole or a part of the abdomen?
- Any nausea or vomiting at this particular time? If vomiting, what are the contents?
- Did it sometimes occur even without food?
- Any change in bowel habits, consistency , color or so?
- Please tell me about any other symptoms you have other than the ones as asked above.

The most probable cause of such a pain can be as follows:
- Subacute intestinal obstruction.
- Less possible causes are: problems related to the gall bladder, pancreas or stomach (as per the present history).

I would suggest you the following:
-Standing X-ray of the abdomen when there is a pain.
-Enteroclysis (Barium study) under fluoroscopy screening.
These should help to find the cause.
Additional tests would be:
Upper GI Endoscopy.
CT scan of the abdomen.

I hope these tests may be done at ER or you may consult your Family Physician or a General Surgeon to discuss and get these tests done to get an early diagnosis so that further management can be planned.

I hope this answer helps you. Please feel free to give the feedback about the questions I asked for, and for any relevant further queries or if you feel that there is a gap-of-communication.

Hi, many thanks for your reply.
Please note below I have tried to answer your additional questions as best as I can.
- Is there associated bloating or distension of the whole or a part of the abdomen?
I feel some kind of tension/pressure on the (middle to down) left side of the abdomen.
- Any nausea or vomiting at this particular time? If vomiting, what are the contents?
I do not vomit but yes I feel sick. Strangely I can eat and not feel sick at all, but sometimes I feel very sick for couple hours.
- Did it sometimes occur even without food?
This is what I am not 100% sure about. I believe sickness only appears when I eat but small pain on the left part of abdomen can go on for days.
- Any change in bowel habits, consistency , color or so?
No. I didn't notice any change.
- Please tell me about any other symptoms you have other than the ones as asked above.
Except the above I think I am more or less feeling fine. I have allergy (food, polen etc.) since I was 5. And I suffer with tremor but that's about it.

If you have any additional comment to my health issue in light of above please let me know. I'd be grateful to hear it.
Thanks a million. XXXXXXX
