Suggest Treatment For A Dislocated Acromioclavicular Joint

default
Posted on Thu, 5 Mar 2015
Question: I recently contacted Ask a Doctor about my questioning an x-ray and I did coo\mmuicate to the physician that ordered the x-ray. his order stated I have a dislocated acromioclavicular joint and the ex-ray shows my ligaments are ruptured from the acromio clavicular joint and ligaments that attaches the clavicle to the coracoid process of the scapula. IO asked the physician to look at the films because he is affilicated with the hospital and he has access to view the films. He told he was not going to look at the films and he is only going to keep record of the Radiologist. I am studying whether the radiologist may be using previous diagnosis codes my physician has used in the past when I had asked for a separate medical claim form for me to submit to my supplemental insurance company that does pay for physician's office visits. What has changed is that I now have health insurance and this insurance company had received the diagnosis codes. I am wanting to find out whether the diagnosis codes represent any sanity diagnosis. Here are the diagnosis codes and I need the correct description of them:

272.4

715.96

600.00

These diagnosis codes were used in my preventative visit on October 3, 2014 along with a service code of 99214 as physician office visit. He is delaying my request for a February 3, 2015 office visit for my supplemental office visit.. This office visit included 99213..

Brief Answer:
Diagnosis codes as per International classification of diseases ICD 9

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail
The US health system still uses the ICD 9 codes for insurance.

You have probably mentioned ICD 9 codes which I have detailed below

272.4     OTHER AND UNSPECIFIED HYPERLIPIDEMIA

715.96     OSTEOARTHROSIS UNSPECIFIED WHETHER GENERALIZED OR LOCALIZED INVOLVING LOWER LEG

600.00     HYPERTROPHY (BENIGN) OF PROSTATE WITHOUT URINARY OBSTRUCTION AND OTHER LOWER URINARY TRACT (LUTS)

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek

Thank you very much. If I knew how to link my CDR I received by my request that has my x-ray films on it for you to see the images I would include it:

It is reassuring that the codes my physician selected for my regular check up represented what the preventive appointment was about. Thank You again. I am sorry I did not reply earlier but, the email went to my spam file. Altough I did refresh my message and assumed it would show that I received an answer I do really appreciate the answer.

I do not like that my physician does not want to look at the films personally because it makes my appeal to my supplemental insurance company more difficult. Although, I can add illustrations from other experts in Orthopedic Medicine but, I am not their patient and I do not know whether the orthopedist I was referred to would support the results of the films as dislocation but, I may ask for a radiologist who has interpreted an MRI of a previous unrelated test I had asked for...

Detailed Answer:
Hi XXXX,
Thanks for writing back with an update.

A patient physician relationship is one based on trust and integrity. If you were my patient and required me to have a second look at your X rays then I would have surely obliged within the confines of a patient doctor commitment. It surely helps to understand the patient problems better when having the X ray film in front of you.

For insurance purpose and to get a better picture, you can always ask the services of the radiologist who reported an earlier unrelated MRI, as long he is authorized to give an opinion to claim insurance.

If you get an opinion from the radiologist who reported the MRI and then request a clinical review by your orthopedist on whether it is a grade 2 or 3 AC tear, that should benefit you.

I am a radiologist myself but unfortunately cant do much as I am not authorized to see your films. Hope your matter with supplement insurance is settled soon and in your benefit.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
