Suggest Treatment For OCD Behaviour

Question: Hello,
My name is XXXX. I have been suffering from a continuous mental / psychiatric / other (which I am not sure) issue. Please help me on this.
Outline:
About in 2008, after I graduated from Johns XXXXXXX University in USA, I started getting weird disturbances that someone was actually controlling me and not allowing me to do focus or concentrate on anything. I was continuously suffering from such a disorder which aggravated and I lost my job due to inactivity or lack of focus or interaction with anyone. I continued to stay alone for a year before I got back to XXXXXXX in 2010. I had to resist the external control which was not allowing me to think / take rest / remember / interact with anyone. I felt (and still feeling) a solid band that presses on my forehead always. I consulted lot of psychiatrists and they prescribed lot of medicines which were of no help (For example I was prescribed Scizopine about five in a day).

Later about a year back, I decided to fight it back myself (stopped all the medications), thinking there is nothing controlling me and focus on my day to day activities with strong determination, now there is a great improvement as I feel relatively better.

But as time is passing really fast that I need to be competent / keep learning / interact better to keep myself stable professionally and also keep up good with my family.

Real challenge is actually remembering things, this solid band of pressure on my forehead is actually stopping me from staying calm (I feel restlessness always), memory issues, "fast breathing", unable to sit in a place for long, unable to focus, unable to interact (If I am talking to someone, while in the discussion I totally LOSE CONTEXT of what I was talking, stay blank and this happens everytime). This can be termed as short term memory loss but I did MRI and other scans and my medical reports are perfectly normal.
I dont know what is going on. Please suggest me on this.

Three diagnoses come to mind here as possibilities: obsessive compulsive disorder, generalized anxiety disorder, or schizophrenia.

The treatment of the first two would be an antidepressant medication such as escitalopram. The treatment of schizophrenia is with an antipsychotic medication such as risperidone.

I recommend consulting with one psychiatrist and sticking with this person to gather a complete history and prescribe medication as indicated.

Hello Dr. H Sheppe,

I just consulted you regarding my psychiatric problem and you also replied to consult you in your private website.

Because I have been taking medications for three years. There is a huge pile of prescriptions.

I will simply text you the medications in reverse chronological order. If you think I need to scan the prescriptions, I can do so in the follow up.

Initially, the doctor I consuldiagnosed it as F.20.0

As such there was no improvements because of medications. But I had few side effects like salivation in bed, excessive drowsiness (that was the primary reason to stop them as I had to work in the mornings).

One quick observation is that immediately after I stopped medication myself, there was an increase in disturbances (I had a fix of sort), but it slowly came down and now the condition is far better than what it used to be during or before medications.

Below is the list of medications (it includes Risperidone - "Risperdal 4mg tablets" and two times injections of "Risperdal Consta 25mg Deep IM":

Hello, and thanks very much for reaching out to my direct private service.

The diagnosis F20.0 confirms my original suspicion that you have schizophrenia. I have reviewed your medication history and list very carefully.

I strongly recommend you resume Sizopin and work closely with your psychiatrist to slowly increase the dose. Schizophrenia is a chronic illness that may do well for a while without medication but will actually need continuous medication treatment to prevent relapse and hospitalization. Sizopin is the most effective medication available for treatment of schizophrenia, but the dose you were last taking (100mg) is likely too low. At a higher dose I would expect full and lasting resolution of symptoms.
