Brief Answer:
Improper occlusion or improper cap

Detailed Answer:
Hi

Thanks for writing in.

As you told there is no problem with RCT but still you have pain on mastication,
it could be due to :-
- improper occlusion in that region due to high point or short cap
- improper cap with flat surface causing slipping of food from occlusal table
- short margins of cap causing food lodgment & resulting in pain

You need to go for proper clinical examination & get it treated accordingly.
Consult endodontist for further treatment plan.

Hope I answered your query

Wishing you good health

Thanks Dr XXXXXXX .But I am looking for the exact cause
1)The pain starts 1)In the RCT performed premolar(performed 2 years back), when I eat chicken and fibres stuck in between gaps .
2)Now for last 4-5 days , (pain)on regular basis in XXXXXXX and evening (when environment is bit cold here in XXXXXXX in november).

I visited the nearby dentist and he after performing the Xray(attached) recommended me to perform RCT again (this time by him only and calculated expense as rs 9000 including crown).
Although, he failed to provide me the exact region where he can see the infection.
please have a look and do help me with ur acumen.
(the attached Xray can be opened in paint).

Many Thanks,
XXXXXX

Brief Answer:
Re-RCT is indicated

Detailed Answer:
Hi

Please reupload your x-ray as its not getting opened. But as per your history of pain there might be some problem in RCT which required Re-RCT.
Either resend the X- Ray or consult endodontist for expert opinion.

Brief Answer:
Abscess formation in periapical area.

Detailed Answer:
Hi

I viewed your reports & there is abscess formation in periapical area. So I would suggest you to go for Re-RCT if you want to save the tooth. Oderwise Extraction is the last option.

Thanks Dr XXXXXXX ..It seems you are available round the clock to help your patients...Your dedication is highly appreciable and helped me a lot.
I need to have one more favor of yours please..
During the time I am gong through the supervision, I have taken the antibiotics(Cipmox500) and it seems the ache has been vanished for now.(Can't say if it has been vanished for ever or temporary basis).
Could you please guide me if I still go for ReRCT (to save the natural tooth) or there is no need of it as there is no ache.

Thanks,
XXXXX
