Hello
Thanks for writing to us with your health concern.
Let me explain the process of medical abortion with pills to you.

While undergoing a medical abortion, one should constantly be under medical supervision.
Let me explain why this is important.
Firstly, the pregnancy should be confirmed by a doctor , preferably with an ultrasound scan.
So it is not just enough to have a positive home pregnancy test, but it is vital to consult a gynecologist and have a pelvic ultrasound scan.
This has two purposes.
It dates the pregnancy accurately, because these abortion pills work only within a narrow frame of time.
Secondly, it confirms that the pregnancy is inside the uterus, rarely, there is a condition called ectopic pregnancy ( pregnancy outside the uterus ) which is very dangerous.

After confirming the pregnancy, tablets are taken by prescription.

Generally 1 tablet of Mifepristone is taken orally, 48 hours later 4 more tablets of Misoprostol are taken orally or inserted vaginally.

Bleeding with cramps starts in a few hours, and lasts heavily for a few days.

Spotting might continue for 15 days or more.

It is compulsory to have a follow up scan after 15 days to establish that the uterus is empty and the abortion is now complete.

Pills are more than 96 % effective in early pregnancy, however rarely there maybe complications such as - heavy bleeding, infection, failure of pills leading to surgical abortion , fainting etc.

Hence it is a must to keep a doctor in the loop throughout.

Misoprostol tablets can be taken orally or vaginally both.

Generally the combination of 1 mifeprin and 4 misoprostol is successful in more than 96 % of cases, and chances of having an incomplete abortion are quite low, less than 5 % in fact.

Also, if a doctor is kept informed, then the requisite actions can be immediately taken.
So yes, medical abortion is quite safe nowadays if done with due precautions.

All the best

Hi doctor
I want to know if termination of pregnancy is legal and once I visit a doc I would necessarily be adviced for med abortion. And will I have problems conceiving again if I go through this. is it preferable to take the pills orally or through vagina.
Thanks
doctor
Answered by Dr. Aarti Abraham (2 hours later)
Brief Answer:
Yes it is legal,

Detailed Answer:
Hello dear.
Yes it is perfectly legal, and once you consult a doctor and express your preference for a termination, a medical abortion will be advised if you fit the criteria for it.
Generally for pregnancies less than 7 weeks, medical abortion is the first option given.
Most women who undergo a medical abortion do not face any complications in future that jeopardizes their fertility.
Chances of severe complications that affect fertility are less than 1 % in fact.
Mifepristone can be taken orally, and the remaining tablets can be taken orally or vaginally, whatever suits you.
Take care.

Hi doctor
I want to know that what is the relevance of time interval between mifepristone and misoprostol. Does increasing or decreasing it effect the efficacy of the medicine. I took mifepristone yesterday at 8 pm . I am experiencing slight cramps in lower abdomen. Can I take misoprostol after 24 or 36 hours or is it mandatory to wait for 48 hours. My exact pregnancy is 25 days old. I also want to know that if I take painkillers after taking misoprostol will it effect the termination in adverse way Since I am very scared thinking about the pain I will undergo.
Thanks
doctor
Answered by Dr. Aarti Abraham (31 minutes later)
Brief Answer:
Can take painkillers.

Detailed Answer:
Hello again.
If you take it earlier than 24 hours, chances of success are less.
Wait for atleast 24 - 36 hours, if not 48 hours.
I hope you had a scan before taking these pills, it is very important as I explained earlier.
you can definitely take painkillers.
Take care.

The scan says Usg findings are suggestive of irregular gestational sac within the endometrial cavity with a small echogenic focus within it ,which could represent fetal pole. However no color flow suggestive cardiac activity is seen withiN the echogenic focus. The mean sac dia is approx 9mm less then 5 weeks . I got the scan done after taking mifipristone. I had misoprostol approx 69 hours after mifipristone. I am scared since I can't feel any significant pain or any bleeding till now. Can I walk or sit or go to washroom after misoprostol
doctor
Answered by Dr. Aarti Abraham (8 minutes later)
Brief Answer:
Wait, you will bleed

Detailed Answer:
Did you bleed anytime in time period between taking the Mifepristone, and getting the ultrasound scan done ?
Why did you take so long to take the Misoprostol, it is to be taken within 48 hours.
Of course you can walk, sit , use the washroom, resume all normal activities.
Dont worry, for sure, you would bleed within a few hours.
Relax.

Hi doctor , I didn't bleed between mifepristone and ultrasound , medicine was delayed coz I waited to see the doc , while I went to the washroom an hour ago I saw little bleeding and clotting happening and cramping has increased a bit. but it is not as much as I heard or read on net. just to inform u I don't bleed too much during my periods neither cramp too much. Is it necessary to bleed too much and cramp too much. Do u think I should take two more tablets of misoprostol to ensure ? I am apprehensive coz as I already told I have a history of incomplete abortion.
doctor
Answered by Dr. Aarti Abraham (16 minutes later)
Brief Answer:
You might not bleed too much.

Detailed Answer:
Hello again.
As it is , it is a very early pregnancy, so the bleeding will be less than what is traditionally expected.
SO dont worry.
Dont take any more tablets right away.
What is your weight ?
And hey, this is the first time you have mentioned an incomplete abortion.
You never told me anything about an incomplete abortion.
Please let me know the details about it right away.

I am 64 kgs and height s 163 cms. My last mtp occured 4 years ago when my hubby was alive . That time had a lot of cramping and very less bleeding ...had to undergo suction..compared to last this time bleeding is ok and cramps are quite less. When do u advice ur patients to have second dose of misoprostol.
doctor
Answered by Dr. Aarti Abraham (17 minutes later)
Brief Answer:
Second dose if given if repeat scan shows retained products.

Detailed Answer:
Okay how many weeks gone were you at that time ?
Did you take the complete set of pills ?
You have to wait for a few days to see if more bleeding happens.
Spotting might continue for a few weeks.
A repeat scan is advised after 15 days to check if the abortion is incomplete.
At that time, if the retained products inside the uterus are insignificant, then a repeat dosage of Misoprostol is given.
If the products are large in amount, a D and C is advised straightaway.
Hope that clears the confusion.
And do not worry, just because you had an unsuccessful abortion earlier, that does not mean it will be incomplete this time also.
Be positive.
It is just 5 weeks pregnancy, so bleeding and cramps will be less, just slightly more than a heavy period.
Take care.
