Does Intake Of Telma-H Cause Electrolyte Imbalance?

Question: Hi Dr,

My mother who is 62 years old got operated on 2nd July for a fracture in leg and plates were implanted in her leg.She was discharged after three days and after 12 days she complained of vomitting,diarrhoea and for four days she could not eat anything.One night she fainted and was imediately admitted in hospital.On diagnosis,it was found that she was suffering from Hyponatremia and Hypokalemia .Her sodium level was 108.She was constantly on Saline and drips for five days.On discharge her sodium level was 131 .She used to take Telma H for BP and doctor said that Telma H could be one reason for sodium loss.After discharge we have been monitoring her Sodium levels and it has come 135,134,134 and 131 and potassium 5.31.Hope the range is normal and nothing to worry.Til how long we need to monitor her sodium n potasdium levels?
Is dere any chance of relapse?
We give her extra salt in bikasul capsule.How long should we give this?
For Bp,Dr has given Telma AM.Hope this medicine is fine and will not contribute to salt loss.
She has been told to take Protien X with milk.For how long should we give her this powder?
We also give her ORS.Hope we can continue this and will not cause a problem

Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
You don't need to monitor her sodium potassium now as it has recovered.
Telma-H can be a contributing factor for her electrolyte imbalances. The main factor remains hospital stay and disturbed diet.

If it has been because of these reasons only then the chances of relapse are minimal.

Don't give her salt capsules now.
Just give her oral rehydration salt solution (ORS) for maintaining her electrolytes.

Telma AM is safe in consideration of electrolyte imbalances.
You can also continue her protein powder.

Thanks for your reply Doctor.Much relieved.

We are giving her ORS Daily .Doctor has told us to give Telma AM at 9 am in the morning so is it before or after breakfast?

How many times should we give her Protein powder and till how long?

She is also taking Betonin Syrup .What is the use of this syrup and till how long should we continue giving this syrup?

Can we monitor her Na and k level.once in a week or month?

Was surgery the main reason of this problem or poor diet?

Can you sugest certain foods which will help her regain strength and also maintain electrolyte balance as she is very weak.

Is dere any need to monitor bp regularly.

In case of implants in fracture,how soon patient can walk as dere is no plaster but only knee cap

Her bones are brittle .How can that be strenghened

Hi. 2 packets or ORS should be consumed in a day and just for 1 week.
Protein powder can be taken 2-3 tsf in milk twice a day.
Betonin is a multivitamin and can be taken for 2-3 months.
You don't need repeated sodium potassium levels done. You can get it once in coming week.
Hospital stay, poor diet, Telma H are all responsible for her electrolyte imbalances.
You can give her banana , coconut water.
Vegetables and dairy products help in maintain bone strength.
The answer when she can walk and all related information would be given by your treating doctor.
I wish her a speedy recovery.

Hi Dr XXXXXXX

Another concern we got her Sodium level tested today also and it again dropped to 129 as against 131 yesterday.Earlier her reports were 134,134 .Why the drop?We are really worried now

We spoke to our doctor and he prescribed us NATRISE 15MG to be given her now .and take extra salt.

Tomm he asked us to get her sodium,potassium and Urine culture done.What is the reason of this drop?as it is not possible to monitor her NA levels daily.

Kindly advise and what is the minimum level of NA which needs immediate medical intervention.

After taking milk she does complain of loose motion and doctor has advised to stop milk .Kindly revert asap

Hi, yes she can be started on Natrise as advised by your doctor.

You can monitor her sodium levels daily for a week, till all settles.

Admission is required if the person has symptoms or sodium levels less then 125.

Yes, milk should be stopped if she has loose motions with it.

Till how long natrise should be continued.

Also what is the cause of this problem as it gets very difficult to monitor her sodium.levels daily.How much time will it take this problem to settle?

Doc has advised to give extra salt in food till how long it should be continued?as she has bp problem also.

We got her Sodium level monitored today and it came out to be 135 today as against 129 yesterday.Is it because of NATRISE 15 MG which she took yesterday.

Doc has advised her to take natrise 15mg for anothrr 3 days.

Is it a serious problem?

ORS and Natrise both help in maintaining the sodium levels.
This problem should settle in a few days, however, if persists, she would need further evaluation by Urine osmolality test, MRI Brain.
