Does Hydrosalpinx Cause Abdominal Pain?

Question: Hello, my doctor finding after seeing him for abdominal pain. I don't understand it. What exactly is causing my abdominal pain? Would the hydrosalpinx be dangerous if untreated or is causing the pain?

FINDINGS:
The uterus is absent.

The right ovary measures 3.4 cm x 1.7 cm x 3.2 cm on endovaginal
images. The architecture and vascular waveforms of the right ovary
are normal. Redemonstrated is a tubular fluid-filled structure
adjacent to the right ovary, likely hydrosalpinx, seen on the prior
examination.

The left ovary measures 1.9 cm x 2.9 cm x 1.6 cm on endovaginal
images. The left ovary is normal architecture and normal vascular
waveforms.

There is no free fluid.

Multiple calcifications or clips are seen in the nearby regions of
the pelvis.

IMPRESSION: Status post hysterectomy.
Normal appearing ovaries. Small right hydrosalpinx, seen in 2012.
Multiple calcifications or surgical clips in the nearby regions of
the pelvis.

In my opinion hydrosalphinx might not be serious always.
But for better suggestions I would like to know following things:-
1)Are you sexually active?
2)Is pain abdomen associated with fever and any altered vaginal discharge?
3)Is this the recent ultrasound report?
4)Why and when was hysterectomy done?

To add, very painful during intercourse and only sometimes my urine smelt line metal.

It might be due to preservation of tubes

Hello dear,thanks for the information.I understand your concern.
In my opinion the indication for the hysterectomy might be pelvic inflammatory disease(PID) as suggested from your history of painful intercourse and associated urinary infection.
The hydrosalphinx seen might be either due to previous PID or due to hysterectomy without removal of tubes and ovaries.
Only hysterectomy is done in younger woman in a view that fallopian tubes are preserved to protect the blood supply to ovaries.
So the tubal secretions might get accumulated leading to hydrosalphinx.
As you have no infection signs like fever you need not worry.It migt resolve spontaneously.If any infection signs develop you might need antiboitics.
If the size is large it might cause complications like torsion but as it is small it might not cause any complications but for pain abdomen.
It might be of more significance in reproductive age woman as it is one of the causes for infertility.
Symptomatic treatment for pain abdomen might give you relief and if the size is increasing as noted by serial ultrasounds then you might need surgery.
Dont worry as it rarely causes any complications.
