My fiancé is 23 years old and has had chronic back pain since he was 17. He has had PT, MRI, lumbar punctures and just about every other test done. he hurts when he moves, works and he gets bad spasms I can visibly see. he hasn t had any injuries and we do not know what to do next. He seems fine some days and worse on others, any advice?
Hello and welcome to HCM, A chronic backache in a young male without a history of trauma is suggestive of arthritis and possibly ankylosing arthritis. Thus, I suggest you to consult an orthopedician for clinical assessment and relevant investigations to rule out ankylosing arthritis. A X-ray and MRI of lower spine are required as first line investigation. If these investigations are suggestive of ankylosing spondylitis, HLA typing by PCR is required to make a confirmed diagnosis. Ankylosing arthritis is an autoimmune disease thus treatment is aimed to inhibit the autoimmune process.
