What Is The Treatment For Chest Pain Due To Lesions In Tricuspid Valve Of Heart And Micro Clots In Lungs?

My brother, who is 35, was admitted to the ICU on 12/25. He presented with chest pain, fever of 103.7, bp was around 60/40. He had relapsed and was using heroin, pain pills and binge drinking. He had a straph infection in his heart, which caused lesions of vegetation on his tricuspid valve. His heart is enlarged. He is swollen all over. He was septic from the infection. His kidneys were shutting down, his liver enzymes were high. He has micro-clots in his lungs, pneumonia, pleurisy. Today, 1/10, a rash (lesions fail to blanch) has shown up on his legs and spread up to his torso. He also is now unable to walk. They have ongoingly monitored his spine for infection and his brain. His liver and kidneys have improved. There is something in his blood that they have not been able to figure out. He has had multiple blood transfusions, platelets and albumin given. His crit is low. What is going on with the rash? Any tests we should be asking for? Anything else you can share?

Un fortunately your brother has infective endocarditis (tricuspidal valve infection which is very common among heroine users) which is a very dangerous disease.

The lesions in the foot are typical for infection spreading from your brother heart to his legs.

The best treatment is a systemic antibiotics regimen and local antibiotics and primary wound care.
