What Is The Best Treatment For Severe Teen Acne?

What is the best thing for moderate to severe teenage acne 17 yo male? We have tried all the over the counter stuff, he has been on oral antibiotics for about two weeks and considering adapolene but it is very expensive, and we have Sooner Care. Also, he has OCD and recently began taking Fluvox to help minimize his scrupulosity OCD. Thanks!

Hi. Thanks for posting your concern at HCM

Acne is a chronic condition and requires regular care upto a certain age by when the acne activity gradually subsides on its own

Specific treatment:

Oral Antibiotics: For inflammatory acne(papules and pustules) i advice my patients to start with one of the oral antibiotics like Azithromycin/ Doxycycline/ Minocycline.

Then there are oral retinoids which can also be prescribed in severe grade acne.

Topical treatment: Is a valuable adjunct to any form of acne treatment. Mainly two classes of drugs are used topically

1)Topical Antibacterials: Either clindamycin 1%/ benzoyl peroxide 2.5% OR Nadifloxacin during the day time.

2)Topical Retinoids: Either Adapalene 0.1%/ Tretinoin 0.025% OR Azaleic acid (10%/20%) at bed time.

Topical treatment regimen especially the one which includes one of the retinoids that i mentioned above also helps to gradually fade away the black spots (post acne hyperpigmentation)(PAH).

General measures form an invaluable part of specific therapy.

General measures:

1)Use a mild cleanser like cetaphil twice daily
2)Avoid all the regular use creams, moisturizers, sunscreens that you may be using for the time being till you are on anti-acne treatment.
3)Avoid taking facials.
4)Avoid picking your acne.
5)Use an Anti-dandruff shampoo since acne is worsened by dandruff.

I would advice that you see a dermatologist for a detailed discussion about the various treatment modalities in your case.
