short answer : your dosage of your medications has to be increased with regular exercises
detailed answer: even by taking glycomet GP 850 both your fasting and postprandial sugar levels are high.
you need to have a strict diet pattern and there must be adequate spacing between each meals.
you must have regular exercises. everyday.
if i were your treating physician i would have changed the diabetic medications i would rather prefer tab. zeformin xr 60 twice daily along with ppg .3 twice daily. for you hope this explains

Diabetologist Dr. M. Vamsi Krishna's  Response
Dear madam/sir i can understand your concern.

I am sorry to hear you have been having difficulties getting your blood sugar in the normal range now. I am very happy you are this conscientious because having a well controlled blood sugar is very important in preventing the onset of diabetic complications.

in my opinion due to long duration of diabetes there might be decreased production of insulin in pancreas.
There is nothing to get alarmed because this is a natural happening in the course of diabetes progression.
i suggest you to undergo some test to see the capacity of pancreas to produce insulin in case the test results comes poor its advisable to use insulin to control your blood sugar.
in case the test result show good to moderate pancreatic reserve i suggest you to consider taking medications like sitagliptin or vildagliptin, this kind of medications are new and they can improve your pancreas function also
The test is Fasting and stimulated C-peptide analysis
diet plan and exercise are most important and need to follow daily for better control of diabetes
