Pathologist and Microbiologist 's  Response
Hello and welcome to HCM,
Urine report shows presence of 10-12 / hpf. This indicates urinary tract infection.
I suggest you to get urine culture.
The electrolyte are within range.
Blood urea nitrogen and creatinine are high and indicates kidney dysfunction.
You need to consult your nephrologist for management.

Thanks and take care

Internal Medicine Specialist Dr. Ashaq Parrey 's  Response
You need an ultrasound to see kidney size if kiddies are small that means you have chronic kidney disease and improvement is less likely. If kidney size is normal the possibility of improvement is good.
