What Does This MRI Report Indicate?

Dear Sir/Madam, I am Prasanta from Bangalore, my sister got Severe pain from thigh to Leg (Right) ; and she does not have any previous medical history,no BP,No Sugar and no diabetic. AS per MRI report, Diffuse disk at L4-L5 level with superimposed posterior annular tear and right paracetral disc extrusion with inferior migration of the disc causing moderate spinal canal with impingement of right traversing root. Doctor recommended for surgery. could you please advise and also let me know if there is non surgery treatment will workout and also need following clarification . 1. How this surgery is critical and What is the risk factors/side effect post surgery 2. After this Surgery, is the pain stops permanently 3. dear careful sister
your patients clinical symptoms and mri report can be correlated. in simple terms human body has vertebral body separated by soft jelly material filled within fibrous ring in the intervertebral space. Due to some reason fibrous ring has broken and jelly material within it has spread into posterior vertebral canal which is compressing right side nerve roots. Due to compression of nerve roots she gets pain in back and right side lower limb. your mri report is not giving any information on canal diameters. if canal diameter is very narrow it may lead to severe pain due to ischaemia of nerve roots.if conservative management with rest and medications is not helping surgery may be required. Alternative is epidural steroid injection which is very helpful I'm such cases. now a days surgery is done with Endoscopy and minimal incisions and patient becomes mobile on same day. Endoscopic discectomy is done with minimal complication in expert hands.
