What Does This CT Scan Report Of Kidney Mean?

I am in 3rd stage kidney failure/renal insufficiency syndrome, unknown etology, although it seems genetic as there are three in a family of 7 diagnosed with same thing. eGFR is 40-45% for past three months. I have recently had a CT scan performed for Right flank pain. Results show: 1) Extrarenal pelvis bilaterally with dilation of extrarenal pelvis and proximal ureter on right; no stones evident. No hydronephrosis bilaterally. 2) Subcentimeter para-aortic nodes. 3) appendicolith or residual contrast in appendix. My questions: 1) What can cause extrarenal pelvis dilation with no evidence of stones? Not misread, as dilation is on one side and not the other. 2) Can nodes be due to infection draining from kidney (Hx of kidney/UDI three months ago with nothing but normal flora cultured). 3) Can appendicolith cause right flank/URQ pain? 4) Any other ideas what could be causing constant right flank pain?

* As per my surgical experience, the CT scan report of kidney is indicative of congenital disorder of renal drainage system in terms of development of extra renal pelvis bilaterally with dilation of proximal ureter and pelvis.

* The renal function in terms of serum creatinine blood urea blood urea nitrogen level BUN / creatinine ratio renal blood flow are very vital as eGFR is only 40-45%

@ It is the developmental anomaly responsible for the extra renal pelvis dilation
@ The nodes are not in relation with infection, but rather reactionary to systemic disease underlying
@ Appendicolith would not pause pain in right upper quadrant, unless it is very high appendix starting from area behind cecum.
