What Does The Following Semen Analysis Report Suggest?

Hi Doctor, Myself Pradeep, Age 29 married one year ago is planning for a baby and we have a problem in conceiving. I had a semen analysis today and following is result.please let me know if I am normal or do I need any treatment. Thank you SEMEN ANALYSIS Time of Collection 12:55 pm Time of Examination 01:55 pm PHYSICAL EXAMINATION Volume 1.0 ml Colour Opaque Greywhite Reaction Alkaline Liquefaction more than 1 Viscosity Normal pH 8.0 Agglutination Present SPERM COUNT 32 million/ml MOTILITY Grade a - Rapid and progressively linear 10 % Grade b - Slow and sluggishly linear 30 % Grade c - Whirling and non-linear 10 % Grade d - Immotile 50 % MICROSCOPY WBC/ Pus cells 8 - 10 /hpf Flagellates ABSENT Absent Epithelial Cells 1 - 2 /hpf Absent Erythrocytes 1 - 2 Absent Amorphous crystal Not found ABNORMALITY Normal 40 %

Thanks for posting in HCM.
I understand your concern.
The following things can be inferred based on the semen analysis report you have enclosed.
1. Volume of 1 ml is towards the lower end of normal range.
2. Colour of opaque greyish white can be considered as normal.
3. Reaction of alkaline nature and pH of 8.0 is normal.
4. Liquification of more than 1 hour is more time taken than normal.
5. Viscosity is normal.
6. Sperm count of 32 million per ml is normal.
7. Sperm motility with 10% rapid and progressively is highly less than normal. Immotile sperms constituting 50% does not seem to be good.
9. Sperm morphology of normal forms constituting 40% is not good.
8. Microscopy with 8 to 10 pus cells is indicating presence of infection.
Hence, it states from the report that there are problems related to sperm motility and normal forms of the sperms. It could be due to the presence of infection and poor seminal nutrition. The fertility factor seems to be low with the above observations.
Kindly take a course of oral antibiotics to overcome infection. Along with it, you need to medications containing L-carnitine, Alpha lipoic acid, CoQ10, Lecithin, Vitamin E, Selenium and Zinc for 3 months, which are known to provide essential nutrients for the sperms.
Kindly get the semen analysis done after 3 months to look for the improvement.
