What Does A Large Mass On Neck Indicate?

Hi. my mom has a large mass on the back of her neck. it is about 4inches wide and 2 and a half inches long. it is hard around the edges and soft in the middle. it is very painful and it feels like it is choking her out. it has been there for 14 years. no tests nor x-rays has been done on it to determine what it is.
