Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

What is your serum creatinine levels, fever and breathing problem may be pneumonia or pulmonary edema in kidney failure due to urine retention or it may be due to anemia or diabetic keto-acidosis.
Until examination is done it is difficult to say what it is. you may be acquired enteric fever or malaria please consult your physician and get it done blood investigation for malaria, complete blood picture, serum creatinine ,malaria chest x-ray then get treated accordingly.

Hope I have answered your query.
Let me know if I can assist you further.
