Hello.....
According to your details it seems to be your are suffering from complications of chronic alcoholism. In this case, patient developed gastritis, Vitamin B12 deficiency, Neuropathy, Lack of concentration and Generalized weakness. So all your symptoms are suggestive of this.
Fist of all you have to undergo some basic lab tests like Complete blood count(CBC) with peripheral blood smear (PBS), Serum vitamin B12 level, Ultrasonography of abdomen to see liver echotexture, portal hypertension and pancreas, UGI endoscopy for gastric ulcer and any esophageal varices.
The disease will be cleared after all such above investigation.
But, in my opinion, according to your clinical details, you are most probably suffering from Macrocytic anemia with Gastritis.
In this case, you can start Cap Esomeperazole + Levosulpride twice a day in empty stomach, Syp Sucralfate 10 ml thrice ad day, Cap Vitamin B complex (containing B1 and B12) twice a day and most important is try to quit or reduce your alcohol consumption as much as you can along with your smoking.

Thank You
