What Causes The Testicle To Shrink?

Why is my left testicle shrinking? I am 71 YO and have blood in my semen. I have a vasectomy. I have no nodules perceptible in my scrotum. Last PSA was 1.74 six months ago and is consistent value with other PSAs in the past----i.e. no elevation of concern. I have no pain.

unilateral testicular atrophy post vasectomy has many differentials.
1. age related changes.
2. exposure to estrogen e.g. liver cirrhosis
3. trauma, alcoholism
4. denervation atrophy
Do Harmonal profile, ultrasound abdomen, testicular Doppler ultrasound to settle the diagnostic dilemma.
