Urologist 's  Response
Hi,
He could be having epididymo-orchitis which basically means inflammation of testicle and its associated structures. this could be due to a urinary infection, which would explain his urinary urgency as well. More often than not, it responds well to antibiotics and anti-inflammatory agents, and scrotal support will reduce the testicular discomfort.
I suggest you get a few basic tests done, which include a urine analysis, a urine culture, and an ultrasound examination of scrotum for evidence of fluid collection. he can be started on broad spectrum antibiotics like Quinolones, and drugs like Solifenacin might reduce his urinary urgency considerably.
