What Causes Severe Pain Below The Rib Cage?

I've been having pain that started in the middle of my stomach. It went from there to my right side underneath my rib cage. I've been to two different hospitals. One hospital told me that my gallbladder was not functioning right and sent me to see a surgeon. I could not go to my appt because I had no way too. Then I went back through the emergency room at a different hospital and they said my gallbladder was fine. What do I do? I'm in so much pain it hurts to sit, stand, lay down, and/or even walk. Now the pain is in my back and underneath my shoulder. Pain rate from 1-10 its a 10.

General & Family Physician 's  Response
Hi, i do care for your concern.
your pain suggest more of an gall bladder related issues.
kindly get done an ultrasound to clarify this issue.
but if you have smoking and drinking habit, then most of the times it may be due to severe duodenal ulcer,
i would advise
to start on drug such an rabikind and continue for one week, in the meantime, get your ultrasound done.
Hope I have answered your question. If you have any further questions I will be happy to help.
