What Causes Severe Fatigue And Swelling In The Legs?

The last few days my husband cant stay awake he is very very lethargic. He is a insulin dependent diabetic and his blood sugar the last few days wont come under 200. He also the last few days has got 4+ edema in his legs feet , from the knee down . we cant get his swelling to go down but him not being able to stay awake scares me . what could be causing him not to be able to stay awake , in the whole day he may be awake 30 mins at a time.
